Práce je prospektivní studie a zabývá se artroskopickou rekonstrukcí rotátorové manžety. Na podkladě výsledků experimentální a klinické části hledá optimální hranici, kdy je z funkčního hlediska indikován vlastní rekonstrukční výkon. Patofyziologické a anatomické aspekty vzniku trhliny manžety jsou uvedeny v úvodu práce. Zásadní příčinou vzniku je subakromiální impingement, který poškozuje šlachu m. supraspinatus. Úpon tohoto svalu bývá narušen jako první a podle rozsahu jeho retrakce je určena závažnost ruptury dle Habermeyera?Pasquiera. Tato klasifikace dělí pacienty do čtyř skupin, neboť každý stupeň vyžaduje specifický přístup i léčebné úkony samotné. Ve studii bylo prospektivně zhodnoceno celkem 250 operovaných pacientů. Možnost funkční rekonstrukce manžety je závislá na stupni degenerace svalu, která byla předoperačně stanovena dle magnetické rezonance, a v experimentální části byl tento parametr srovnán s peroperačně odebranými, histologickými vzorky svalů. Výsledky klinické i experimentální části dokládají, že stupeň degenerace svalu stanovený dle magnetické rezonance dobře koreluje s histologickým rozborem vzorků. Rekonstrukční operace na trhlině rotátorové manžety je indikována nejvýše do III. stupně degenerace svalu dle Goutalliera. Velikost retrakce prasklé rotátorové manžety není přímo závislá na stupni degenerace svalové tkáně.The work is a prospective study concerned with arthroscopic rotator cuff repairs. Based on results of the experimental and clinical parts, it is aimed at searching for an optimal threshold at which, from a functional perspective, the reconstructive procedure itself is indicated. The pathophysiological and anatomical aspects of rotator cuff tears are presented in the introduction. The underlying cause is subacromial impingement producing damage to the supraspinatus muscle tendon. First, the insertion of this muscle is damaged. Based on the extent of its retraction, the severity of the rupture is determined using the Habermeyer?Pasquier classification. This is used to categorize patients into four groups, with each grade requiring specific approach and therapeutic procedures. The prospective study comprised 250 patients who underwent surgery. The possibility of functional reconstruction of the cuff depends on the degree of muscle degeneration as preoperatively determined by magnetic resonance imaging. In the experimental part, this parameter was compared with intraoperatively collected histology muscle tissue samples. Results of both the clinical and experimental parts confirm that the degree of muscle degeneration determined by magnetic resonance imaging correlates well with histological analysis of the samples. Reconstructive surgery for rotator cuff tears is indicated up to Goutallier grade III muscle degeneration. The extent of retraction of a torn rotator cuff is not directly dependent on the degree of muscle tissue degeneration.
