Úvod: Pacienti po cévní mozkové příhodě (CMP) často trpí foot drop syndromem, kdy během švihové fáze chůze nedochází k dorsiflexi, a tím se chůze zpomaluje a hrozí riziko pádu. Funkční elektrostimulace je jedním ze způsobů, jak zlepšit parametry chůze. Cíl: Ověřit, zda používání FES zlepší parametry chůze, jako je rychlost chůze a kvalita chůze. Dále také zjistit, zda má rehabilitace s FES vliv na spasticitu a soběstačnost pacienta. Metodika: Studie se zúčastnilo 10 probandů v subakutním až chronickém stádiu po CMP. Všichni pacienti trpěli spastickou parézou dorsiflektorů a spasticitou plantiflexorů. Před zahájením terapie byla všem odebrána anamnéza a provedeny klinické testy a byly vyplněny dotazníky. Po absolvování 3týdenní hospitalizace s intenzivní rehabilitací a tréninkem chůze s FES bylo provedeno výstupní vyšetření. Hodnotila se zde rychlost chůze, subjektivní vnímání kvality chůze, soběstačnost, spasticita a paréza. Výsledky: Při výstupním vyšetření vykazovali pacienti statisticky významné (p <0,05) zlepšení oproti vstupnímu vyšetření. Projevilo se to ve všech měřených oblastech, tj.: rychlost chůze, subjektivní vnímání kvality chůze, soběstačnost, spasticita a paréza. Závěr: Kombinace fyzioterapie a tréninku chůze s FES pozitivně ovlivňuje poruchy chůze u pacientů v subakutním a chronickém stadiu po CMP.Introduction: Patients after stroke often suffer from foot drop syndrome, where dorsiflexion is not achieved during the swing phase of gait, slowing down the gait and increasing the risk of falling. Functional electrical stimulation (FES) is one way to improve gait parameters. Aim: To test whether the use of FES will improve gait parameters such as gait speed and gait quality. Also, to determine if rehabilitation with FES has an effect on spasticity and self-sufficiency of the patient. Methods: 10 probands in the subacute to chronic stage after stroke participated in the study. All patients had spastic dorsiflexor paresis and plantiflexor spasticity. Before starting therapy, a medical history was taken and clinical tests were performed and questionnaires were completed by all of them. After a 3-week hospitalization with intensive rehabilitation and gait training with FES, an exit examination was performed. Gait speed, self-reported gait quality, self-sufficiency, spasticity, and paresis were assessed. Results: At the exit examination, patients showed statistically significant (p < 0.05) improvement compared to the entrance examination. This was evident in all measured areas, i.e.: gait speed, subjective perception of gait quality, self-sufficiency, spasticity, and paresis. Conclusion: The combination of physiotherapy and gait training with FES positively affects gait disturbances in patients in the subacute and chronic stages after stroke.
