Poruchy chůze po cévní mozkové příhodě (CMP) limitují pacienty v běžných denních činnostech, a proto je zlepšení chůze jedním z primárních cílů rehabilitace. Rehabilitace chůze podléhá klinickému obrazu pacienta, biomechanickým charakteristikách jeho chůze, a také individuálním potřebám pacienta. Otázkou je, který rehabilitační postup se v kontextu současné evidence based medicine jeví jako efektivní pro zlepšení chůze u pacientů po CMP. Na základě klíčových slov: cévní mozková příhoda, chůze, krokový cyklus, hemiparetická chůze, rehabilitace, proprioceptivní neuromuskulární facilitace, Bobath koncept, trénink chůze, a jejich anglických ekvivalentů: stroke, gait, gait cycle, hemiparetic gait, rehabilitation, Proprioceptive Neuromuscular Facilitation, Bobath Concept, gait training, byly v online databázích PubMed, Google Scholar, Science Direct a SpringerLink vyhledávány relevantní články publikované v letech 2001-2021. Pro tvorbu práce bylo celkově použito 51 zdrojů, z toho 42 článků v anglickém jazyce v elektronické podobě, 7 knih v českém jazyce a 2 knihy v anglickém jazyce. Z výsledků studií vyplývá, že existuje celá řada rehabilitačních postupů, které využívají jak tradiční terapii založenou na neurofacilitačních přístupech, tak klasický trénink chůze s využitím např. vodního prostředí, či aerobního a silového tréninku. Výsledky také poukazují na fakt, že žádná terapie není lepší než jiná a že je třeba terapii přizpůsobit individuálně každému pacientovi.Gait disturbances after stroke limit patients in normal daily activities, and therefore improving gait is one of the primary goals of rehabilitation. Gait rehabilitation is subject to the clinical picture of the patient, the biomechanical characteristics of their gait, and the individual needs of the patient. The question is which rehabilitation procedure appears to be effective in improving gait in patients after stroke in the context of current evidence-based medicine. Based on the keywords: stroke, gait, step cycle, hemiparetic gait, rehabilitation, proprioceptive neuromuscular facilitation, Bobath concept, gait training, the online databases PubMed, Google Scholar, Science Direct and SpringerLink were searched for relevant articles published between 2001 and 2021. A total of 51 sources were used for the work, including 42 articles in English in electronic form, 7 books in Czech language and 2 books in English language. The results of the studies show that there are a number of rehabilitation procedures that use both traditional therapy based on neuro-facilitation approaches, as well as classical gait training using, for example, aquatic environments or aerobic and strength training. The results also highlight the fact that no one therapy is better than another and that therapy needs to be tailored to the individual patient.
