Diplomová práce se zabývá bezprostředním efektem respirační fyzioterapie na dechové parametry u pacientů na umělé plicní ventilaci. Teoretická část obsahuje informace o umělé plicní ventilaci, indikační kritéria, nejčastěji využívané formy a možné komplikace. Dále je v ní pojednáno o respirační fyzioterapii na jednotkách intenzivní péče, roli fyzioterapeuta na JIP a využívaných technikách respirační fyzioterapie v České republice i ve světě. Hlavním cílem výzkumu bylo objektivně zhodnotit bezprostřední efekt respirační fyzioterapie u pacientů na umělé plicní ventilaci. Do testovaného souboru bylo zařazeno 13 pacinetů s akutním respiračním selháním ventilovaných režimem BiPAP, od kterých bylo získáno celkově 31 měření. Hodnoty respiračních parametrů se odečítaly před a bezprostředně po 20 minutové terapii z monitoru ventilátoru a výsledků Astrupova vyšetření krve. Terapie obsahovala myofasciální ošetření hrudníku, ošetření bránice pod processus xiphoideus a dolními žeberními oblouky, kontaktní dýchání lokalizované brániční dýchání, odporované dýchání, vibrační techniky a reflexní terapii podle Vojty 1. fázi reflexního otáčení. Výsledky ukazují, že bezprostředně po respirační fyzioterapii vzrostly hodnoty dechového objemu, minutového objemu a dynamické plicní compliance, současně byl zaznamenán signifikantní pokles odporu dýchacích cest a plicního zkratu.The Master's thesis explores an immediate effect of respiratory physiotherapy on wind parametres of mechanically ventilated patients. The theoretical part contains information about mechanical ventilation , criteria for its indication, its most used forms and possible side effects. Further on it describes respiratory physiotherapy on Intensive Care Units (ICUs), the role of a physiotherapist in ICUs and the exploited techniques of respiratory physiotherapy in Czech republic and the whole world. The main goal of this research was to objectively valorize the immediate effect of respiratory physiotherapy on mechanically ventilated patients. The tested set was formed by 13 patients with an acute respiratory failure ventilated by BiPAP from whom we gathered 31 measurements overall. The values of respiratory parametres were deducted before and immediately after the 20 minutes therapy from the ventilator monitor and from the results of an Astrup blood test. The therapy consisted of myofascial therapy of thorax, the diaphragm treatment under the xiphoid process and below the rib cage, contact breathing localised diaphragm breathing, resisted breathing, vibration techniques and reflexive Vojta therapy 1. phase of reflexive turning. The results suggest an increase of breathing volumes, an minute volume and the values of dynamic lung compliance immediately after the respiratory intervention. A significant decline of airways resistance and pulmonary shunt was registered as well.
