Chronická lymfocytární leukémie (CLL) je klonální onemocnění B lymfocytů, které jsou z důvodu narušení mechanismů apoptózy akumulovány v periferní krvi, kostní dření, játrech, slezině a v dalších orgánech. Přesný mechanismus vzniku CLL není dodnes zcela znám a je předmětem výzkumu. CLL častěji postihuje starší populaci (50 let a více). Diplomová práce podává přehled nejdůležitějších prognostických ukazatelů u CLL pacientů a zaměřuje se především na molekulárně genetické markery. Experimentální část mé diplomové práce je zaměřena na detekci mutací v genech BIRC3 a NOTCH1 u rizikových pacientů s CLL a to jak u refrakterních pacientů, kteří nereagují na provedenou léčbu, tak u pacientů, u kterých je onemocnění aktivní. Ve své práci jsem využila metody molekulární biologie - PCR, Sangerovo sekvenování, alternativní PCR metoda ARMS (amplifikační refrakční mutační systém) a HRM (High resolution melting) analýza, umožňující detekci změny v jedné bázi. Cílem diplomové práce bylo zavést HRM analýzu pro detekci mutací v genu BIRC3 u pacientů s CLL a zjistit její uplatnění v rutinní praxi na Hemato-onkologické klinice LF a FN Olomouc. Praktická analýza nádorové DNA od souboru nemocných ukázala, že HRM analýza poskytne informace o přítomnosti či absenci mutace v genu BIRC3 a může být vhodnou screeningovou metodou pro více pacientů najednou, ale musí být vždy ověřena i Sangerovým sekvenováním. Sangerovo sekvenování je tak stále plnohodnotnou metodou v rutině molekulárně biologických vyšetření.The chronic lymphocytar leukemia (CLL) is a clonal disease of B lymphocytes which are acumulated in peripheral blood, bone marrow, liver, spleen or other organs thanks to the disorder of apopthosis mechanism. The accurate mechanism of the CLL genesis has not been entirely known up to present day and is the item of investigation. CLL affects elderly population (50 years and more) more often. This diploma thesis gives an overwiev of the most important prognostics indicators in CLL patients and it is focused especially on molecular-genetic markers. The experimental part of my final thesis is focused on the detection of mutations in BIRC3 and NOTCH1 genes in risk patients with CLL in refractery patients, who do not react on the performed treatment, and also in patients where the disorder is active. In my thesis I used the method of molecular biology - PCR, Sanger sequencing, alternative PCR method ARMS (Amplification refraktory mutation system) and HRM (High resolution melting) analysis which enables the detection of changes in one bases. The aim of my diploma thesis was the inplementation of HRM analysis in the detection of mutations in BIRC3 gene in patients with CLL and the detection of its application in the routine praxis in the Hemato-oncological clinics in Olomouc University Hospital. The practical analysis of cancer DNA in a group of patients demonstrated that the HRM analysis gives information about the presence or absence of the mutation in BIRC3 gene and it can be an appropriate screeinng method for more patients at the same time, but it must always be checked by Sanger´s sequencing. Sanger´s sequencing remains a fully-fledged method in the routine of molecular-biological examination.
