Hemato-onkologická onemocnění, mezi které se řadí akutní myeloidní leukemie a myelodysplastický syndrom jsou velmi častým maligním onemocněními. Diagnóza těchto onemocnění je založena na vyšetření kostní dřeně, což je značně invazivní vyšetření. Proto je snaha o nalezení vhodných markerů, jejichž hodnoty by byly dostatečně prokazatelné i ze vzorků periferní krve a mohly by přispět ke zpřesnění nebo urychlení diagnostiky MDS. Dalším důvodem pro detekci metylačních změn u myelodysplastického syndromu je nedávné zavedení léčby pomocí demetylačních látek u tohoto onemocnění. V rámci diplomové práce byly analyzovány promotorové úseky potenciálního tumor-supresorového genu RIL, genu pro inhibitor cyklin-dependentních kináz 2B (CDKN2B) p15, genu BLU, který je důležitý v procesu apoptózy a u genu RASSF1, u kterého se předpokládájí změny exprese v nádorových onemocněních v důsledku hypermetylace (Boumber et al., 2007; Park et al., 2013). Pro analýzu jsme použili jak běžně používanou metodu metylačně specifická PCR, tak metodu pyrosekvenování. Výsledky naznačují, že pyrosekvenování je daleko přesnější a citlivější metodický postup než klasická metoda metylačně specifická PCR. Metylační změny genů BLU a RASSF1 zjištěné u pacientů s MDS jsou minimální při porovnání s výsledky získanými od zdravého jedince. U sledovaných úseků genů p15 a RIL jsme detekovali významné metylační změny v porovnání se zdravým jedincem. Následná expresní analýza genu RIL, ukazuje na možnou korelaci mezi metylaci promotorové oblasti tohoto genu a jeho expresí. Geny p15 a především RIL by mohly být v budoucnu využity jak pro diagnostiku MDS a AML tak i pro určení prognózy u pacientů s MDS.Hemato-oncological diseases like acute myeloid leukemia and myelodysplastic syndrome, are very often malignant diseases. Diagnostic of this diseases is based on an examination of the bone narow, which is very invasive examination. Because of this, efforts are focused on finding new suitable markers provided for of the periferal blood samples contributing to specification and speeding of the MDS diagnosis. Another reason for DNA methylation detection in the myelodysplastic syndrome patients, is a new approach to the treatment based on applying of demethylation agents. In this thesis, we analyzed promoter region of the potential tumor-suppresor gene RIL, the inhibior of cyklin dependent kinase 2B (CDKN2B) p15, BLU gene with an important function for appoptosis and RASSF1 gene in which is assumed an change of the expresion in some types of tumors caused by its hypermetylation (Park et al., 2013; Boumber et al., 2007). For methylation analysis we used Methylation specific PCR and following method of pyroseqencing. Our results indicate that pyroseqencing is more accurate and sensitive methodical approach than methylation specific PCR. DNA methylation in the BLU and RASSF1 genes detected in MDS patients comparable with healthy person was unsignificant. In the genes p15 and RIL, we detected significant changes of the methylation level by comparison with helthy control. Following expression analysis of the RIL gene showed possible correlation between promoter methylation level of this gene and his expression level. Finally, the p15 and even more the RIL genes could be useful markers for diagnosis of the MDS and AML diseases as well as for the MDS prognosis.
