Disertační práce se věnuje endoskopické transnazální chirurgii selárních a paraselárních patologii. V obecné části této disertační práce je popisována historie, chirurgická anatomie a operační přístupy s důrazem na transnazální endoskopickou techniku. Shrnuty jsou i nejčastější selární patologie, jejich klinická prezentace, diagnostika a terapie. Vědecká část dizertační práce je rozdělena do třech kapitol. 1. První kaptitola hodnotí výsledky hormonálních funkcí v terapii afunkčních makroadenomů v zavislosti na zvolené operační technice (mikroskopická versus endoskopická transnazální transfenoidální technika). 2. Druhou kapitolu tvoří epidemiologická studie, která hodnotí četnost, typ nádoru, klinickou prezentaci, terapii a prognózu u pacientů s diagnózou metastázy ve tkáni hypofýzy. 3. Ve třetí kapitole jsou prezentovány dvě kazuistiky, se kterými se autor setkal ve své klinické praxi. Jedná se o transsfenoidální meningoencefalokélu a nádor z granulárních buněk v oblasti infundibula a neurohypofýzy.The dissertation pursues endoscopic transnasal surgery of the sellar lesion. In the general part of this thesis is described history, surgical anatomy and surgical approaches with emphasis on transnasal endoscopic technique. The most common sellar pathology, their clinical presentation, diagnosis and therapy are summarized. The scientific part of the dissertation is divided into three chapters. 1. The first chapter evaluates the results of hormonal functions in the treatment of non-function macroadenomas in relation to selected surgical techniques (microscopic versus endoscopic transnasal transsphenoidal technique). 2. The second chapter is an epidemiological study that evaluates the frequency, type of tumor, clinical presentation, therapy and prognosis in patients diagnosed with metastasis in pituitary tissue. 3. In the third chapter are presented two case reports, which the author encountered in his clinical practice. It is a transsphenoidal meningoencephalocele and a granulary cell tumor of the infundibulum and neurohypophysis.
