Úvod: Graded Motor Imagery (GMI) neboli stupňovaná představa pohybu je neinvazivní imaginativní metoda, efektivní zejména k léčbě bolesti. Jedná se o terapeutický přístup sloužící k optimalizaci kortikální reorganizace, tedy správného vnímání tělesného schématu, které je u chronických bolestivých stavů vlivem maladaptivních neuroplastických změn narušeno. Cíl: Cílem této diplomové práce bylo zhodnotit, má-li GMI vliv na snížení bolesti u chronických algických stavů v oblasti ramene. Metodika: Do výzkumné části diplomové práce bylo zařazeno 15 probandů s chronickými bolestmi ramene (> 3 měsíce). Hodnocení bolesti bylo provedeno pomocí dotazníků, jež probandi vyplnili před zahájením terapie a po jejím ukončení. Konkrétně se jednalo o: numerickou škálu bolesti (NŠB), dotazník interference bolesti s denními aktivitami (DIBDA), zkrácenou verzi dotazníku bolesti McGillovy Univerzity (SF-MPQ), dotazník kineziofobie (Tampa Scale for Kinesiophobia - TSK) a revidovaný dotazník copingu bolesti (DCB-R). K vyhodnocení správnosti a času rozpoznání 80 karet v rámci tréninku stranového rozlišení sloužily údaje ze záznamového archu. Výsledky: Vlivem terapie nastalo statisticky významné snížení intenzity bolesti na numerické škále, zmírnění bolesti při provádění běžných denních činností, a také snížení senzorické i afektivní složky bolesti, a tudíž i celkového skóre bolesti. Nebyl však prokázán signifikantní vliv GMI na snížení strachu z bolestivého pohybu (kineziofobie) ani na zlepšení vypořádání se s bolestí (copingu bolesti). Následkem tréninku stranového rozlišení došlo k signifikantnímu zvýšení počtu správně rozpoznaných karet a snížení času nutného k určení strany. Závěr: GMI vedla k signifikantnímu snížení bolesti a zlepšení stranového rozlišení u pacientů s chronickými bolestmi ramene.Introduction: Graded Motor imagery (GMI) is a non-invasive imaginative method that is particularly effective for treating pain. It is a therapeutic approach used to optimize cortical reorganization - the correct perception of the body schema. Due to maladaptive neuroplastic changes, cortical reorganization is disturbed in patients with chronic pain conditions. Aim: The aim of this thesis was to reveal whether GMI may result in the decrease of pain in patients with chronic algic conditions in the shoulder area. Methods: The research section of the thesis was conducted with the help of 15 respondents suffering from chronic shoulder pain (> 3 months). Pain assessment was performed using questionnaires completed by respondents before and after therapy. Used questionnaires are the following: Numeric Pain Scale (NPS), Pain Interference with Daily Activities Questionnaire (DIBDA), the short-form of the McGill Pain Questionnaire (SF-MPQ), the Tampa Scale for Kinesiophobia (TSK) and the revised form of the Coping with Pain Questionnaire (DCB-R). In case of laterality training, data recording sheets were used to assess accuracy and recognition time of 80 cards. Results: The treatment resulted in a statistically significant reduction in pain intensity on a numerical scale, pain relief during the performance of daily activities, and a reduction in both the sensory and affective components of pain, and thus in the overall pain score. However, no significant effect of GMI on reducing the fear of painful movement (kinesiophobia) or improving pain coping was demonstrated. As a consequence of laterality training, there was a significant increase in the number of correctly recognized cards and a decrease in the time required to determine the side. Conclusion: GMI therapy resulted in significant pain reduction in patients with chronic shoulder pain, as well as in an increase of their laterality recognition ability.
