Tématem této diplomové práce bylo použití skiagrafie a výpočetní tomografie při diagnostice poranění krční páteře u kraniotraumat. Cílem práce bylo porovnat dva diagnostické postupy CT hlavy a krční páteře a CT hlavy a RTG krční páteře z hlediska procentuální četnosti zachycených fraktur a radiační zátěže. Dále se práce zabývala tím, zda lze z informací na žádankách k CT hlavy a krční páteře vysledovat signifikantní rizikové faktory predikující nález fraktury krční páteře. Výzkum proběhl retrospektivní formou s využitím žádanek k vyšetření, obrazové dokumentace a radiologického popisu na vzorku 1508 pacientů, kteří podstoupili ve FN Brno v roce 2019 zobrazovací vyšetření. Výsledky ukazují, že CT detekovalo fraktury na krční páteři signifikantně častěji než skiagrafie. Signifikantně vyšší efektivní dávku z vyšetření obdrží pacient při CT hlavy a krční páteře, a to průměrně až o 2 mSv více než u CT hlavy a RTG krční páteře. Ze žádanky a z vyšetření CT hlavy a krční páteře byly vysledovány signifikantní rizikové faktory predikující nález fraktury krční páteře, mezi které patří přítomnost krčního límce na CT, bolest v oblasti krční páteře a přítomnost parestezií či plegie.The topic the diploma thesis discuses is the use of the radiography and Computed Tomography in the diagnosis of cervical spine injury associated with cranial trauma. The aim of the work is to compare two diagnostic approaches of a head plus neck CT scan and a head CT scan plus neck radiography considering the percentage of reported fractures and radiation dose. The thesis also examined whether the informatik on CT scan requets can predict the presence of cervical spine fracture. The research was made by a retrospective approach using the regest forms, image documentation and radiology reports of 1508 patients who went through an imaging examination in The University Hospital Brno in 2019. The results showed that CT was able to identify cervical spine fractures significantly more often than the radiography. Significantly higher effective dose from the examination received patients undergoing the CT scan of head and cervical spine than those undergoing head CT scan and cervical spine radiography with the difference reaching up to 2 mSv. The informatik on regest forms for head and cervical spine CT showed some risk factors predicting fracture of the cervical spine. The risk factors included the presence of a neck collar during the CT, neck pain at the cervical spine area and the presence of paraesthesia or plegia.
