Infekce lidskými papilomaviry (HPV) je jednou z nejrozšířenějších sexuálně přenosných infekcí u mužů i žen po celém světě. Přetrvávající infekce vysoce rizikovými HPV viry může vést ke vzniku maligních lézí anogenitálního traktu, u žen především ke karcinomu cervixu. Neustálý vývoj screeningových metod a zlepšování jejich senzitivity a specificity, je klíčový pro včasnou diagnostiku přednádorových a nádorových změn v oblasti cervixu. Cílem této práce byla validace nové detekční metody pro HPV68, která by sloužila k potvrzení pozitivity vzorku pro tento genotyp v případech, kdy se rutinně požívané CE-IVD HPV DNA detekční metody Cobas? 4800 HPV Test a PapilloCheck? HPV-Screening test neshodují. Mezi další cíle práce patřilo objasnění příčiny rozdílnosti výsledků získaných při detekci HPV68 pomocí dvou výše uvedených CE-IVD HPV detekčních metod. Výsledky získané pomocí těchto dvou detekčních metod byly porovnány s údaji získanými pomocí nově navržené kvantitativní multiplex real-time PCR, která byla využita k potvrzení přítomnosti infekce genotypem HPV68 ve 42 cervikálních a cervikovaginálních stěrech, a ve vzorku cervikálního karcinomu. Ve všech vzorcích byla určena virální nálož a forma HPV infekce (epizomální nebo integrovaná). Cílovými geny multiplex real-time PCR byly HPV68 geny E2 a E6, které hrají důležitou roli při integraci viru do genomu hostitele a gen pro GAPDH, který byl použit jako interní kontrola amplifikace. Oblast genu E6 byla dále využita k rozlišení subtypů HPV68a a HPV68b v jednotlivých vzorcích pomocí touchdown PCR s následnou analýzou křivek tání (HRM). Výsledky multiplex real-time PCR i PapilloCheck? HPV-Screening testu prokázaly přítomnost HPV68 ve všech studovaných vzorcích. Cobas? 4800 HPV Test detekoval přítomnost HPV68 pouze u 39,4 % vzorků. Falešně negativní výsledek byl zaznamenán u více než poloviny vzorků infikovaných pouze genotypem HPV68. Jako příčina rozdílnosti výsledků byla identifikovaná nízká senzitivita Cobas? 4800 HPV Testu pro subtyp HPV68a. Cobas? 4800 HPV Test je rutinně používáným HPV screeningovým testem, který je zároveň používán v mnoha studiích. Prevalence HPV68 může být díky jeho nížší senzitivitě vůči HPV68a falešně nižší.Human papillomavirus (HPV) infection is one of the most common sexually transmitted diseases in men and women. Persistent HPV infection with high-risk HPV can lead to malignant transformations of anogenital tract, primarily to cervical cancer in women. Incessant development of new screening strategies and increase in their sensitivity and specificity is key for early detection of precancerous and cancerous changes of the cervix area. The aim of this study was validation of a new detection method for HPV68, which could be used to confirm the occurance of this genotype in case of the discrepancy between the commonly used CE-IVD HPV DNA detection methods Cobas? 4800 HPV Test and PapilloCheck? HPV-Screening test. The aim was also to clarify the cause of the discrepancy between the results obtained with the two mentioned CE-IVD HPV detection methods. Results obtained from the two methods were then compared with results from the newly designed quantitative multiplex real-time PCR, which was used to confirm the occurance of HPV68 infection in 42 cervical and cervicovaginal swab samples, and a sample of cervical cancer. All the samples were tested for viral load and were examined on the type of infection (episomal or integrated). Targeted genes for the multiplex real-time PCR were E2 and E6, which play an important role in the integration of the virus to the host's genome and the gene for GAPDH which was used as internal control of amplification. Detection of the E6 gene region was also used to distinguish the two subtypes HPV68a and HPV68b using touchdown PCR followed by high resolution melting (HRM). The results obtained from both, quantitative multiplex real-time PCR and PapilloCheck? HPV-Screening test proved the presence of HPV68 in all studied samples. However, Cobas? 4800 HPV Test detected HPV68 only in 39,4 % of the studied samples. The false negative result was detected in more than half of HPV68 single type infection cases due to its lower sensitivity for HPV68a. Prevalence of HPV68 genotype could be falsely lower as the Cobas? 4800 HPV Test is routinely used HPV screening test and is used in the majority of published studies.
