Příčiny mužské neplodnosti mohou být vrozené nebo získané. Nejčastěji se jedná o kombinaci obojího. Geneticky podmíněné selhání nebo zhoršení spermatogeneze může být ovlivněno až 150 geny. Většina prací této problematiky je zaměřena na kandidátní geny z oblasti AZF. Jedním z dalších kandidátních genů je TSPY. Gen TSPY je gen kódující testes specifický protein. Jedná se o multikopiový gen. Počet kopií se pohybuje od 30 do 60. Gen má přibližně 2,8 kb a obsahuje 6 exonů a 5 intronů. Předpokládanými funkcemi je úloha proliferačního faktoru ve spermatogenezi a regulace genové exprese. TSPY je fyziologicky exprimován v tkáni varlete. Dále byla zjištěna exprese v patologických tkáních (prekurzory tumorů, karcinomy in situ, seminomy a dalších). Cílem práce bylo porovnání sekvenčních polymorfismů v genu TSPY, kvantifikace relativního počtu repetic a posouzení exprese genu ve fyziologických a patologických tkáních u neplodných mužů, pacientů se seminomem, Y pozitivních pacientek s Turnerovým syndromem a kontrolního souboru. Jednotlivé vzorky byly vyšetřeny pomocí PCR, sekvenační analýzy, PCR in situ a imunofluorescence a statisticky vyhodnoceny. Významné změny byly nalezeny v prvním exonu. U poloviny neplodných mužů a kontrol bylo sledováno 26 nejčastějších polymorfismů s podobným patternem. U druhé poloviny neplodných mužů byly nalezeny velmi důležité změny generující stop kodon v první třetině exonu 1. Předčasné ukončení transkripce by zkrátilo protein o 78 %. Tento typ změny nebyl nalezen v kontrolním souboru. Korelace mezi spermiogramem a změnami, jež vedou ke změně čtecího rámce nebyla nalezena, stejně jako rozdělení mužů u nichž se vyskytovaly delece a inzerce bylo náhodné. Změny nalezené v prvním exonu genu TSPY by mohly značně poškodit proces spermatogeneze. Tyto poznatky mohou značně rozšířit naše znalosti o molekulárně genetických příčinách mužské neplodnosti.The causes of male infertility can be congenital or acquired. Most often it is a combination of both. Genetically determined failure or deterioration of spermatogenesis may be affected up to 150 genes. Most of these studies are focused on AZF candidate genes. Another candidate gene is TSPY, which encodes the testis-specific protein. It is a multi-copy gene. Number of copies varies from 30 to 60. The gene has approximately 2.8 kb and contains 6 exons and 5 introns. There are expected functions: the role of proliferative factor in spermatogenesis and the gene expression regulation. TSPY is expressed in testes tissue. There was described expression in pathological tissue too (tumors, carcinoma in situ, seminoma etc). The aim of this study was to compare sequence polymorphisms in the gene TSPY, quantify the copy number and gene expression assessment in physiological and pathological tissue in infertile men, patients with seminoma, Y-positive patients with Turner syndrome and controls. There were analyzed 75 infertile men and 31 fertile controls using PCR, conventional sequencing analysis, PCR in situ a immunofluorescence. The most remarkable changes were found in the 1st exon only. In one half of the both infertile men and fertile controls, the most frequent finding was 26 SNPs with a similar pattern. In the other half we found highly relevant changes, generating a stop codon in the first third of exon 1. Early termination cut down the protein by 78 %. This kind of change was not found in the fertile controls. No correlation was found between the spermiogram and the changes leading to the stop codon. The distribution of men with deletions, insertion and higher gene copy number was not statistically different. The changes found in exon 1 in infertile men could fundamentally affect the process of spermatogenesis. These findings could significantly enhance our understanding of the molecular-genetic causes of male infertility.
