V disertační práci jsme se zabývali incidencí pooperačních bolestí v krku po tracheální intubaci a po zavedení laryngeální masky (LMA). Cílem bylo zjistit četnost výskytu a rizikových faktorů pro rozvoj pooperační bolesti v krku po celkové anestezii na našem pracovišti. Součástí práce bylo posoudit schopnost anesteziologa klinicky predikovat stupeň naplnění obturační manžety laryngeální masky. Vyšetřovali jsme pacienty, kteří podstoupili elektivní operační výkon v celkové anestezii na operačních sálech chirurgie, traumatologie, urologie, gynekologie, plastické a estetické chirurgie. Studie byla rozdělena do tří fází. V první fázi byli pacienti dotazováni na bolest v krku v den operace, 1. a případně 3. pooperační den. Při bolestech přetrvávajících 3. den jsme provedli otorinolaryngologické vyšetření. V druhé fázi jsme vyšetřili pacienty s uniformním zajištěním dýchacích cest laryngeální maskou. V závěru jsem si ověřila, zda navržená opatření ovlivnila výskyt pooperační bolesti v krku. Ve třetí fázi lékař zjišťoval spontánní stesky pacientů první pooperační den na příslušném oddělení. Soubor byl statisticky srovnatelný se souborem v první fázi, identické bylo i zanášení dat. Během studie jsme z dokumentace dospávacího pokoje, příslušného oddělení a anesteziologického záznamu čerpali demografické údaje, způsob zajištění dýchacích cest, velikosti intubačních rourek a LMA, způsob vedení anestezie a podaná léčiva, která mohla ovlivnit výskyt pooperační bolesti v krku. Podařilo se nám prokázat, že způsob zajištění dýchacích cest a použitá lubrikancia významně ovlivňují četnost pooperačních bolestí v krku. Výskyt je rovněž častější při zavádění laryngeální masky s plně vyfouknutou manžetou, při použití N2O a při tlaku v manžetě LMA nad 60 cm H2O. Doporučujeme k lubrikaci pomůcek zajišťujících dýchací cesty používat inertní gel na vodní bázi a manometrem kontrolovat stupeň naplnění obturační manžety.In our thesis, we analysed the incidence of postoperative sore throat after tracheal intubation and after laryngeal mask insertion (LMA). The aim of the study was to determine the incidence and risk factors of postoperative sore throat development after general anesthesia at our department. Our work also included assessment of the anesthesiologist's ability to predict the degree of inflation of the LMA cuff. Our study consisted of patients who underwent an elective surgical procedure under general anesthesia in the operating room of surgery, traumatology, urology, gynecology, plastic and aesthetic surgery. We divided the study into three phases. Patients were questioned about sore throat pain on the day of the operation, on the 1. postoperative day and in some cases on the 3. postoperative day in the first phase. In patients with pain persisting after the 3. postoperative day, an otorhinolaryngological examination was performed. In the second phase patients were examined who all had the same method of airway management using a laryngeal mask. In conclusion we studied whether the proposed measures influenced the incidence of postoperative sore throat. In the third phase, on the first postoperative day, the anesthesiologist interviewed the patients regarding their condition at their corresponding wards. The patient set was statistically comparable to the patient set in phase I, data collection was also identical. During the study, we used documentation from the postoperative ward, the corresponding patient wards and anesthesiology records to collect data including demographical data, method of airway management, size of intubation tubes and LMAs, anesthesia management, and medication administered, which may have an effect on the incidence of postoperative sore throat. We could show that the method of airway management and the use of lubricants significantly affect the frequency of postoperative sore throat. The incidence is also higher after intubation using LMA with a fully deflated cuff, when using nitrous oxide and when the LMA cuff pressure exceeds 60 cm H2O. We recommend using inert water-based gels to lubricate airway management instruments and monitoring the degree of inflation of the LMA cuff by manometer.
