Tato bakalářská práce se zabývá podílem krevních destiček na hemostázu. Řada patofyziologických stavů a nemocí lidského organismu způsobuje patologicky sníženou nebo zvýšenou funkci krevních destiček. Krevní destičky jsou důležitým činitelem při rozvoji arteriální trombózy a aterosklerózy, které se mohou manifestovat ve formě ischemické choroby srdeční a cévních mozkových příhod. Ateroskleróza patří mezi nejčastější příčiny smrti ve světě. Teoretická část bakalářské práce pojednává o morfologii a funkci krevních destiček. Zvláštní důraz je kladen na jejich základní funkční vlastnost, kterou je schopnost agregace při poškození cévní stěny a následnou aktivaci hemokoagulace. Teoretická část popisuje prevenci vzniku krevních sraženin na základě účinku protidestičkové léčby, která inhibuje agregaci. Experimentální část se věnuje měření agregace destiček principiálně novou impedanční metodou z plné krve na agregometru Multiplate analyzer. Pro tento přístroj bylo stanoveno referenční rozmezí jednotlivých induktorů agregace na základě naměřených dat na skupině zdravých dárců krve. Následně byla stanovena in vitro inhibiční aktivita kyseliny acetylsalicylové (nejčastěji používaného antiagregancia) pomocí vhodného induktoru. Takto byla experimentálně zjišťována odpověď na antiagregační léčbu pomocí agregační schopnosti destiček, což je možné využít k monitorování individuální odpovědi na léčbu.This bachelor´s thesis deals with the impact of blood platelets on hemostatis. A number of patho-physiological states and diseases of the human organism are caused by pathologically reduced or increased function of blood platelets. Blood platelets are a substantial factor in the development of arterial thrombosis and atherosclerosis, which can be manifested in a form of ischemic heart disease and cerebrovascular accidents. Atherosclerosis is among the most frequent causes of death in the world. The theoretical part of this bachelor´s thesis is devoted to morphology and the function of blood platelets. Particular emphasis is placed on their primary functional feature, which is the ability to aggregate when a vein wall is damaged, followed by hemocoagulation activation. The theoretical part describes the prevention of blood clots using anti-platelet treatment, which inhibits the aggregation. The experimental part contains measurements of platelet aggregation with a principly new method which uses whole blood in an aggregometer (Multiplate analyser). Reference ranges of each aggregation indicator were specified for this device using the measured data gathered from healthy blood donors. Subsequently, the inhibiting activity of acethylsalicyl acid (the most frequestly used anti-aggregant) was specified in vitro with the support of an appropriate inductor. In this manner, the response to anti-aggregation treatment as supported by the aggregation ability of blood platelets was tested, which is useful in monitoring individual responses to treatment.
