Úvod: Chůze je základním pohybovým mechanismem potřebným k udržení samostatnosti a nezávislosti. Poruchy chůze jsou u pacientů po cévní mozkové příhodě jedním z nejčastějších problémů. Cíl: Zhodnotit chůzi u pacientů po cévní mozkové příhodě (CMP) snímáním změn svalové aktivity vybraných svalů v obraze povrchové elektromyografie při chůzi na páse a po chodbě před začátkem rehabilitace (1. měření) a po jejím ukončení (2. měření). Metodika: Měření se zúčastnilo 8 probandů v subakutní fázi po CMP (2 ženy, 6 mužů), jejichž průměrný věk byl 60 let (? 10,21), výška 178,625 cm (? 6,435) a váha 88,625 kg (? 26,827), kteří měli poruchu chůze jako následek CMP. Všichni z probandů byli schopni samostatné chůze bez pomůcek a neměli kognitivní deficit. Každý proband se účastnil dvou měření, první proběhlo v rámci vstupního vyšetření před začátkem terapie, druhé při ukončení poslední den hospitalizace. V rámci jednoho měření byla měřena svalová aktivita při chůzi na chodbě a na páse pomocí elektrod povrchové elektromyografie (EMG) z těchto svalů: m. tibialis anterior, m. gastrocnemius medialis, m. rectus femoris a m. biceps femoris bilaterálně. Výsledky: Došlo k signifikantnímu snížení aktivity mezi 1. a 2. měřením chůze na chodbě u neparetického m. biceps femoris (p=0,036). K signifikantnímu rozdílu mezi paretickou a neparetickou dolní končetinou došlo u m. gastrocnemius medialis při 2. měření na páse (p=0,028) a 1. měření na chodbě (p=0,049). Mezi 1. a 2. měřením chůze na chodbě nebyly shledány žádné významné změny. Závěr: V této práci byly prokázány signifikantní změny ve svalové aktivitě během chůze v rámci srovnání chůze před zahájením intenzivní fyzioterapie a po jejím ukončení, i při srovnání paretické a neparetické dolní končetiny. Povrchová EMG nabízí možnost detekovat změny svalové aktivity reflektující kontrolu pohybu a objektivizovat tak efekt rehabilitační léčbyIntroduction: Walking is the basic movement mechanism needed to maintain independence and autonomy. Walking disorders are one of the most common problems in patients after a stroke. Aim: To evaluate walking in patients after stroke by sensing changes in muscle activity of selected muscles in terms of surface electromyography when walking on the treadmill and on the ground before the start of rehabilitation (1st measurement) and after its completion (2nd measurement). Methods: 8 subjects were examined in the subacute phase after stroke (2 females, 6 males), with mean age 60 years (? 10.21), height 178.625 cm (? 6.435) and weight 88.625 kg (? 26.827), with gait disorder as a result of a stroke. All of the probands were able to walk independently without aids and did not have a cognitive deficit. Each proband participated in two measurements; the first took place as part of the initial examination before the start of therapy, the second was at the end on the last day of hospitalization. In every measurement, muscle activity was measured while walking in the corridor and on the treadmill using surface electromyography (EMG) electrodes from the following muscles: tibialis anterior, gastrocnemius medialis, rectus femoris and biceps femoris bilaterally. Results: Statistical results showed a significant decrease in muscle activity between the 1st and 2nd measurements of walking in the corridor in the non-paretic m. biceps femoris (p = 0.036). There was a significant difference between the paretic and non-paretic lower limbs in m. gastrocnemius medialis at the 2nd measurement on the treadmill (p = 0.028) and the 1st measurement on the corridor (p = 0.049). No significant changes were found between the 1st and 2nd measurements of walking in the corridor. Conclusion: In this study, significant changes in muscle activity during walking were demonstrated in the comparison of walking before the start of intensive physiotherapy and after its termination, as well as in the comparison of paretic and non-paretic lower limbs. Surface EMG offers the possibility to detect changes in muscle activity reflecting control of movement and thus objectify the effect of rehabilitation treatment.
