Infekce krevního řečiště a sepse představují život ohrožující stav. Rozhodující pro přežití pacienta je rychlá identifikace etiologického agens, určení citlivosti a nasazení cílené antibiotické terapie. V současné době rostoucího počtu rezistentních kmenů bakterií má rychlé určení antimikrobní citlivosti obzvláště velký význam. V rámci diplomové práce byla otestována home-made metoda přímé identifikace bakteriálních kmenů z pozitivních hemokultur pomocí MALDI-TOF MS. Výsledky identifikace byly porovnány s referenčními diagnostickými postupy, jako jsou mikroskopie a identifikace z bakteriálních kultur izolovaných na kultivačních půdách. Dále byla otestována metoda rychlého stanovení antimikrobní citlivosti (RAST) a její porovnání s referenčním postupem stanovení citlivosti pomocí diluční mikrometody. Metoda přímé identifikace z pozitivních hemokultur pomocí MALDI-TOF MS doplněná metodou rychlého stanovení citlivosti na antibiotika (RAST) umožnila rychlejší získání potřebných výsledků v porovnání s referenčními metodami a je tak velkým přínosem, který zvyšuje pravděpodobnost úspěšné terapie pacientů v sepsi.Blood stream infections are life threatening. To ensure the best chance of patient's survival, the most vital are fast identification of pathogenic microorganism, antibiotic susceptibility testing and application of effective antibiotic therapy. With increasing antimicrobial resistance it is especially important to assess antimicrobial susceptibility. In the thesis, the rapid direct identification of bacteria from positive blood cultures was tested using MALDI-TOF MS. The results were compared to standard identification methods: microscopy and identification of bacterial cultures isolated on growth medium. Next, the rapid antimicrobial susceptibility testing (RAST) was performed and compared to standard broth microdilution method. The combination of direct identification from positive blood cultures together with rapid antimicrobial susceptibility testing (RAST) shortened time to results as compared to reference methods. As such, these methods show potential to increase likelihood of successful treatment of patients in sepsis.Blood stream infections are life threatening.
