Tématem disertační práce je návrh postupu monitorování účinku antiagregační terapie pomocí funkčních agregačních testů a molekulárně biologického stanovení polymorfismů receptorů krevních detiček. Teoretická část disertační práce je zaměřena na popis fyziologie hemostázy, kde shrnuje základy hematopoézy zejména megakaryocytární linie a následně přibližuje procesy související s krevní destičkou jako je např. adheze, aktivace, degranulace a hlavně agregace. Následně je popsán mechanismus ovlivnění hemostázy antitrombotickou léčbou, která je dělena na antikoagulační, trombolytickou a zejména antiagregační. Zvláštní důraz je kladen na monitorování antiagregeční terapie, ať už z laboratorního nebo klinického pohledu. Klinické dopady selhání antiagregační terapie jsou popsány na vysoce rizikové skupině mladých pacientů s akutním infarktem myokardu (AIM), kde lze předpokládat významný vliv genetických predisponujících faktorů. Experimentální část disertační práce se věnuje monitorování protidestičkové léčby pomocí dvou funkčních agregačních testů (optické transmisní agregometrie a impedanční agregometorie). Jako cílová riziková skupina byl hodnocen soubor 124 pacientů s diagnózou AIM. U pacientů rezistentních na anopyrin byla následně provedena in vitro inkubace s kyselinou acetylosalicylovou, pro zjištění příčin selhání protidestičkové léčby. V neposlední řadě byla zjišťována frekvence výskytu polymorfismů destičkových receptorů COX-1 (rs10306114), P2Y12 C34T (rs6785930), P2Y12 T744C (rs2046934) GP1bA (rs2243093) a také GP6 (rs1613662) možné genetické příčiny neúčinnosti antiagregační léčby (aspirinová či thienopyridinová rezistence). K vyšetření polymorfismů destičkových receptorů byla využita metoda real-time PCR s následnou analýzou křivek tání.This thesis deals with monitoring the effects of antiplatelet therapy using two functional aggregation tests that are correlated with molecular biological determination of platelet polymorphisms. The theoretical part of the thesis is first focused on the description of the physiology of hemostasis, which summarizes the basics of hematopoiesis in particular megakaryocytic line and then approaches the processes associated with platelet such as adhesion, activation and degranulation mainly aggregations. Antithrombotic therapy is here divided into anticoagulant, thrombolytic and antiplatelet particular. Special emphasis is placed on monitoring antiaggregation therapy, both in terms of possible technologies used or followed in terms of risk group of patients with acute myocardial infarction (AMI), a disease that is described as one of the major causes of morbidity and mortality in developed countries. The experimental part of the thesis deals with monitoring antiplatelet therapy with two functional aggregation test (optical transmission aggregometry and impedance aggregometry). The target risk group was taken up file 124 patients with a diagnosis of AMI. In patients resistant to aspirin was subsequently performed in vitro incubation with acetylsalicylic acid, to determine the causes of failure of antiplatelet therapy. Finally, the measured frequency of platelet gene polymorphisms COX-1 (rs10306114), P2Y12 C34T (rs6785930), P2Y12 T744C (rs2046934) GP1bA (rs2243093) and GP6 (rs1613662) as a genetic cause of inefficiency antiplatelet therapy (aspirin or thienopyridin resistance) . To investigate polymorphisms of platelet receptors, the method of real-time PCR followed by melting curve analysis.
