Gonartróza je limitující pro pacienty při vykonávání běžných denních aktivit. V případě neúspěchu konzervativní léčby je dalším možným řešením jak pacientům, zvláště ve vysokém stupni postižení kloubu, ulevit od bolesti a ztuhlosti kolene, chirurgické řešení pomocí totální endoprotézy. Cílem této práce bylo zhodnotit vliv totální endoprotézy kolenního kloubu na kinematiku chůze a srovnat jej se stavem před operací. Výzkumný soubor tvořilo celkem 8 probandů, 7 žen a 1 muž, průměrný věk všech probandů byl 66,25?4,63 let. U těchto probandů byla diagnostikována mediální gonartróza II. až III. st. stupně podle RTG dle Kellgrena a Lawrence. K porovnání získaných hodnot kinematických parametrů byl použit Wilcoxonův párový test a znaménkový test. Pro testování hypotéz byla stanovena hladina statistické významnosti = 0,05. Statisticky významný rozdíl (p=0,02) byl nalezen při jednooporové fázi kroku u časoprostorových parametrů obou končetin celkově. Ve stejné skupině byl také zjištěn statisticky významný rozdíl u fáze dvojí opory (p=0,03). U časoprostorových parametrů obou těchto testů byla prodloužena doba opory. Při měření úhlových parametrů vyšly statisticky významné hodnoty v hlezenním kloubu u maximální plantární flexe na konci stojné fáze (p=0,05), u celkového rozsahu pohybu v sagitální rovině (p=0,05) opět u všech skupin. Na neoperované končetině v hlezenním kloubu při maximální plantární flexi na konci stojné fáze (p=0,04). V úhlových parametrech bylo také statisticky významné zvětšení celkového rozsahu pohybu v sagitální rovině v hlezenním kloubu (p=0,02) u obou končetin dohromady. U neoperované končetiny se statisticky významně zvětšil rozsah maximální plantární flexe na konci stojné fáze (p=0,03) v hlezenním kloubu, v kolenním kloubu se zvětšil celkový rozsah pohybu v sagitální rovině (p=0,05) a deprese pánve ve švihové fázi chůze (p=0,03). Totální endoprotéza kolenního kloubu v našem výzkumu statisticky významně ovlivnila úhlové parametry na neoperované dolní končetině. Dále byly statisticky významné hodnoty zjištěny pro úhlové a časoprostorové parametry při chůzi celkově.Knee arthritis is limiting for patients in performing of regular daily activities. In case of conservative treatment failure, a total knee replacement is another possible solution to relieve both pain and stiffness of the knee, particularly in the high degree of joint involvement. The aim of this work is to evaluate the effect of total knee joint endoprosthesis on the kinematics of walking and compare it to the pre-operation condition. The research file consisted of 8 probands, namely 7 women and 1 man, while the average age of all probands was 66.25 ? 4.63 years. For these probands, the medial osteoarthritis of the knee joint grade II and III was diagnosed using the X-ray according to Kellgren and Lawrence. The Wilcoxon signed-rank test was used for comparison of obtained values of kinematic parameters. For hypothesis testing, the level of statistical significance was determined at = 0.05. A statistically significant difference (p = 0.02) was found in the single limb stance of the step in time-space parameters in all groups. In the same group, a statistically significant difference was also found in the double limb stance (p = 0.03). The support stance was prolonged for time-space parameters of both these tests. When measuring the angle parameters, statistically significant values were reported in ankle joint at the maximum plantar flexion at the end of the standing phase (p = 0.05), for the entire range of motion in the sagittal plane (p = 0.05) in all groups again. At the non-operated limb, in the ankle joint at maximum plantar flexion at the end of the standing phase (p = 0.04). In the angular parameters, there was also a statistically significant increase in the overall range of motion in the sagittal plane in the ankle joint (p = 0.02) in all groups. At the non-operated limb, the range of maximum plantar flexion at the end of the standing phase (p = 0.03) in the ankle joint has increased significantly. The knee joint showed an increased overall range of motion in the sagittal plane (p = 0.05) and depression of the pelvis in the swinging stance of walking (p = 0.03). The total knee joint endoprosthesis significantly influenced angular parameters on the non-operated lower limb and angular walking parameters in total in our research. Statistically significant values were found for time-space parameters in walking generally.
