Úvod: Skapulohumerální rytmus ovlivňuje postavení kloubní jamky glenohumerálního kloubu. To má vliv na kloubní kongruenci, výchozí postavení periartikulárních svalů, stabilitu a rozsah pohybu v ramenním kloubu. Dysfunkce skapulohumerálního rytmu se zpravidla objevuje u postižení ramenního pletence. Jelikož se jedná o funkční problematiku, předpokládáme efekt rehabilitační léčby. Cíl: Zhodnotit efekt rehabilitace skapulohumerálního rytmu u pacientů s impingement syndromem. Metodika: Studie se zúčastnilo 35 pacientů s impingement syndromem. Pacienti podstoupili 6týdenní ambulantní rehabilitaci se zaměřením na obnovu skapulohumerálního rytmu. K objektivnímu zhodnocení efektu léčby byl měřen rozsah pohybu v ramenním kloubu do flexe a abdukce pomocí goniometru. Subjektivně byla hodnocena bolest dle Visuální analogové škály bolesti a soběstačnost pacienta v běžných denních činnostech prostřednictvím dotazníku ADLEIR Score. Data byla odebírána při vstupním vyšetření, ihned po první terapii a po 6týdenní individuální rehabilitaci. Výsledky: Došlo k signifikantnímu zvýšení rozsahu pohybu do flexe a abdukce v ramenním kloubu a snížení bolesti, jak po první terapii, tak po 6týdenní rehabilitaci (p < 0,05). Po 6týdenní rehabilitaci došlo také ke zlepšení soběstačnosti pacienta (p < 0,05). Závěr: Cílená rehabilitační léčba skapulohumerálního rytmu má vliv na zvýšení rozsahu pohybu ramenního kloubu, snížení bolesti i zlepšení soběstačnosti pacienta s impingement syndromem.Introduction: Scapulohumeral rhythm effects position of articular fossa of glenohumeral joint. That influence joint congruence, initial position of periarticular muscles, stability and range of motion of the shoulder. Scapulohumeral rhythm dysfunction usually appears within shoulder girdle lesions. Due to the functional character of the problematice we assume effect of rehabilitaion treatment. Objective: To evaluate effect of rehabilitation of scapulohumeral rhythm in impingement syndrome patients. Methodology: 35 patients participated in this study. They received 6 weeks of ambulant rehabilitation with focus on scapulohumeral rhythm recovery. The range of motion to flexion and abduction was measured with goniometer to objectively evaluate the effect of the treatment. Visual analogy scale was used to subjectively evaluate the pain. The ADLEIR Score questionnaire was used to evaluate the independece of the patient in activities of daily living. The data were taken during initial examination, immedaitely after first therapy and after 6 weeks of individual rehabilitation. Results: There was a significant increase of range of motion to flexion and abduction of the shoulder and decrease of pain, both after first therapy and after 6 weeks of rehabilitation, as well (p < 0,05). There was also increase of patient´s independence after 6 weeks of rehabilitation (p < 0,05) Conclusion: Targeted rehabilitation treatment of scapulohumeral rhythm has effect on increased range of motion of the shoulder, decreased pain and improved independence of patients with impingement syndrome.
