Autonomní nervový systém je u Parkinsonovy nemoci (PN) postižen časně, dokonce již v premotorické fázi onemocnění. Autonomní dysfunkce u PN (včetně dalších non-motorických příznaků) jsou závažným problémem, který výrazně přispívá k disabilitě pacientů a zhoršuje tak kvalitu jejich života. Je proto důležité na tyto příznaky myslet, zavčas rozpoznat jejich přítomnost a adekvátně zasáhnout terapeuticky. Cílem naší práce bylo na tuto problematiku nejen upozornit, ale i vnést do českého prostředí jednoduchý nástroj ke včasnému záchytu autonomních poruch u PN, a dále se pokusit pomocí vlastního výzkumu přispět k včasné diagnostice jak vlastní PN, tak zvláště ortostatické hypotenze (OH) u tohoto onemocnění. Pouze včasná diagnóza totiž vede ke včasnému zahájení léčby a tím zkvalitnění života pacientů. Ve snaze zkvalitnit záchyt autonomních poruch u pacientů s PN v České republice jsme se rozhodli provést český překlad Autonomic Scale for Outcomes in Parkinson's Disease (SCOPA-AUT). Jedná se o jednoduchý, časově nenáročný dotazník, který postihuje všechny hlavní oblasti autonomních poruch u pacientů s PN. Nejprve byl dotazník přeložen, poté byla dotazníkovou formou hodnocena srozumitelnost české verze (tzv. "cognitive debriefing") v cílové skupině, tedy u pacientů trpících PN. V další části práce jsme se zaměřili na chromogranin A (CgA) a jeho využití v diagnostice PN. Vzhledem k jeho výskytu v katecholaminergních neuronech jsme vyslovili hypotézu, že by změna jeho hladiny v likvoru mohla odrážet patologii katecholaminergních neuronů, což by mohlo být spojeno s narušením kardiovaskulárních regulačních funkcí. Zjištění abnormní hladiny CgA již v časných fázích PN nás motivovalo k rozšíření souboru pacientů a korelaci hladiny CgA se změnou krevního tlaku po vertikalizaci na nakloněné rovině, tedy na nástroji soužícím mimo jiné k posuzování ortostatické hypotenze (OH). Statistická analýza prokázala pozitivní korelaci mezi hladinou CgA v likvoru a změnou diastolického krevního tlaku. Hladina CgA v likvoru byla signifikantně nižší u pacientů s OH ve srovnání s pacienty s PN, kteří netrpěli OH. Provedené studie by tak mohly napomoci k objasnění mechanismu rozvoje OH u PN. Zjištění, že pacienti trpící OH vykazují nižší hladinu chromograninu A v likvoru než pacienti bez OH, by mohlo mít i praktický dopad. Mohlo by upozornit lékaře na zvýšené riziko OH u jejich pacientů, což by vedlo ke včasnému rozpoznání OH a zahájení léčby, a tím k redukci pádů a zlepšení kvality života pacientů.The autonomic nervous system in Parkinson's disease (PD) is affected early in the course of the disease, even in the premotor phase. Autonomic dysfunction in PD (and other non-motor symptoms) is a serious problem contributing to the disability of patients and decreasing quality of life. Therefore, it is important to search for these symptoms from the early stage of the disease to start adequate treatment. The aim of our work was 1) to call attention to this fact; 2) to translate a reliable, simple and fast autonomic questionnaire, which could be useful in daily practise; 3) to contribute to the undersanding of the pathogenesis of orthostatic hypotension (OH) in PD and to help in the early diagnosis of this condition. In an attempt to help with the early diagnosis of autonomic dysfunction in Czech PD patients, we translated the Autonomic Scale for Outcomes in Parkinson's Disease (SCOPA-AUT). This questionnaire is easy to perform, not time consuming, and evaluates the main areas of autonomic dysfunction in PD. After translation we tested the comprehensibility (cognitive debriefing) of the Czech version by administrating the questionnaire to PD patients recruited from all three Parkinson centres in the Czech Republic. In the second part of our work, we focused on chomogranin A (CgA). CgA is a peptide stored and co-released from the vesicles together with catecholamines. We hypothesised that in PD, the loss of catecholaminergic cells may be associated with decreased CSF CgA levels and is clinically associated with decreased sympathoexcitatory responses resulting in symptoms of orthostatic intolerance. The aim of the first part of the study was to establish the level of CgA in a cohort of treatment-na?ve patients with early stage PD. We found decreased levels of CgA in patients compared to control group. The aim of the second part of the study was to test the abovementioned hypothesis by correlating CSF CgA with blood pressure changes during the upright position on a tilt table. A correlation analysis showed a significantly positive correlation between CSF CgA level and diastolic blood pressure change. There were significantly lower levels of CSF CgA in patients with OH compared to patients without OH. Our studies may help to clarify the mechanism of orthostatic dysregulation in PD. A decreased CSF CgA level may be useful in predicting OH in the early stage of PD. This would be helpful to physicians in recognizing early symptoms of orthostatic intolerance and for starting adequate treatment leading to an improvement in the quality of life of patients suffering from PD.
