IgA nefropatie je nejčastější primární glomerulonefritida. Do její etiopatogeneze jsou zapojeny mezangiální depa imunitních komplexů, v nichž se jako antigen uplatňuje abnormálně glykosylované polymerní IgA1. Makromolekulární imunitní komplexy nejsou efektivně odstraňovány játry a jejich depozice v mezangiu ledvin stimuluje proliferaci mezangiálních buněk a zvýšenou tvorbu proteinů extracelulární matrix, což v konečném důsledku vede ke snížení až ztrátě renálních funkcí. Až třetina pacientů s IgA nefropatií dospěje v průběhu 20 let od diagnózy k nezvratnému selhání ledvin. Teoretická část diplomové práce shrnuje současné poznatky o molekulárních mechanismech vzniku IgA nefropatie, tvorbě a biologické aktivitě cirkulujících imunitních komplexů, genetických faktorech a současných možnostech léčby tohoto onemocnění. Experimentální část práce se zabývá stanovením proliferační odpovědi tkáňové kultury mezangiálních buněk po stimulaci séry pacientů s IgA nefropatií a kontrolními séry. Pomocí metod inkorporace 3H-thymidinu a intracelulárního barvení fluorochromem CFSE byla stanovena schopnost sér pacientů s IgA nefropatií a kontrol stimulovat proliferaci mezangiálních buněk in vitro. Nebyl pozorován signifikantní rozdíl v proliferační aktivitě sér pacientů s IgA nefropatií a kontrolní skupinou. Metodou ELISA byla stanovena vyšší koncentrace celkového IgA i množství cirkulujících imunokomplexů u pacientů s IgA nefropatií. Byla stanovena proliferační aktivita séra pacienta s IgA nefropatií po frakcionaci, která ukázala přítomnost stimulačních a inhibičních imunitních komplexů.IgA nephropathy is the most common primary glomerulonephritis. In its etiopathogenesis is caused by mesangial deposition of immune complexes containing aberrantly glycosylated polymeric IgA1. Macromolecular immune complexes are not effectively eliminated by the liver and its deposition in the renal mesangium stimulates proliferation of mesangial cells and extracellular matrix proteins expansion leading to a decrease or eventually loss of kidney function. Within 20 years from diagnosis up to a one third of patients with IgA nephropathy developes irreversible end stage renal failure. The theoretical part of diploma thesis summarizes the current knowledge about the molecular mechanisms involved in IgA nephropathy, formation and biological activity of circulating immune complexes, genetic factors and current treatment options for this disease. The experimental part deals with the determination of proliferative responses of cultured mesangial cells after stimulation with sera from patients with IgA nephropathy and control sera. The ability of sera from patients with IgA nephropathy and controls to stimulate proliferation of mesangial cells in vitro was determined by 3H-thymidine incorporation assay and by CFSE intracellular staining. There was no significant difference in the proliferative activity of serum from IgA nephropathy patients and controls. Using ELISA approach, a higher concentration of total IgA and circulating immune complexes was determined in patients with IgA nephropathy. Further, it was determined proliferative activity of molecular size-fractionated serum from a patient with IgA nephropathy, which showed the presence of stimulatory and inhibitory immune complexes.
