Akutní pankreatitida (AP) je i v současné době, přes veškerý pokrok diagnostiky i léčby, včetně všech možností intenzivní péče, závažným onemocněním. Mortalita těžkých forem AP sice poklesla, ale stále se pohybuje kolem 10%. Co nejčasnější stratifikace nemocných a predikce závažného a komplikovaného průběhu umožňuje cílenou terapii šitou na míru. Důležitým faktorem pro prognózu onemocnění je infekce pankreatické nekrózy, která s sebou nese dramatický vzestup morbidity i mortality a vyžaduje odlišný terapeutický přístup. Předkládaná disertační práce se zaměřuje na dvě oblasti využití laboratorních vyšetření u akutní pankreatitidy, kterými jsou: a) diferenciální diagnostika sterilní a infikované nekrózy b) časná predikce závažnosti průběhu akutní pankreatitidy Pro využití v diferenciální diagnostice sterilní a infikované nekrózy byl studován interleukin-6, v časné predikci závažnosti AP byly zkoumány klasické zánětlivé ukazatele C-reaktivní protein, prokalcitonin a interleukin-6 ve srovnání s adipokiny adiponektinem,fibroblastovým růstovým faktorem-21 a adipocytárním vazebným proteinem pro mastné kyseliny. Analyzovány byly klasické zánětlivé markeryDespite all the progress of diagnosis and treatment including the possibility of in intensive care acute pancreatitis (AP) is still a serious disease. Mortality of severe forms decreased, but still hovers around 10%. Soon stratification of patients and prediction of complications enables targeted and tailored therapy. Another important factor for the prognosis of the disease is the infection of pancreatic necrosis, which carries a dramatic increase in morbidity and mortality and requires a different therapeutic approach. The thesis focuses on two areas of use of laboratory biomarkers in acute pancreatitis: a) differential diagnosis of sterile and infected necrosis b) early prediction of pancreatitis severity For use in the differential diagnosis of sterile and infected necrosis interleukin-6 was studied and classic inflammatory indicators C-reactive protein, procalcitonin and ínterleukin-6 in comparison with adipokines adiponectin, fibroblast growth factor-21 and adipocyte-fatty acids binding protein were studied for the early prediction of severity of AP.
