Autoři ve studii sledují subjektivní a objektivní efekt operace zlepšující nosní průchodnost (septoplastika a turbinoplastika) na spánkové potíže pacienta. Jednalo se o skupinu 28 pacientů se subjektivní nosní neprůchodností s nálezem anatomické deformity nosní přepážky a/nebo hypertrofie dolních skořep, kteří současně udávali pravidelné chrápání. K operaci bylo přistupováno v případě přetrvávání potíží a lokálního nálezu po konzervativní terapii lokálními nazálními kortikosteroidy. Porovnáním předoperačních a pooperačních vyšetření limitovanou polygrafií byl u všech pacientů potvrzen OSA, z toho u 2 pacientů lehkého, u 14 pacientů středního a u 12 pacientů těžkého stupně. Pooperačně došlo k signifikantnímu subjektivnímu i objektivnímu zlepšení nosní průchodnosti dle porovnání výsledků přední rinomanometrie (p=0,0065). Nicméně i přes zlepšenou nosní průchodnost nedošlo k redukci respiračních událostí, AHI (Apnoe/hypopnoe index) totiž nebyl signifikantně změnen (p=0,0792). Vyšetřením subjektivního efektu operace bylo zjištěno signifikantní snížení chrápání dle sebehodnotící analogové škály 1-10. Naopak, ke zlepšení denní spavosti dle Epworthské škály spavosti pooperačně nedošlo. Operační řešení nosní průchodnosti tudíž nelze považovat na základě zjištěného za léčebnou metodu OSA, protože nedochází pooperačně k redukci respiračních spánkových událostí, nicméně indikací k septoplastice a turbinoplastice je subjektivně problematické nosní dýchání (s objektivním nálezem anatomické překážky nosních průduchů). Pooperační zlepšení nosního dýchání je totiž subjektivně pacientem dobře vnímáno a dle našich výsledků lze očekávat i redukci chrápání. Dalším zdůvodněním operace nosu je i následná léčba indikovaných pacientů CPAP s nazální aplikací - odstraněním překážky nosního průduchu dochází ke snížení aplikačního tlaku a logicky lze dle literatury očekávat zlepšení tolerance CPAP. To se bohužel v našem souboru nepodařilo prokázat (z 8 indikovaných pacientů pouze 3 pacienti tolerovali léčbu). Lze to zdůvodnit nízkým počtem pacientů a dalšími nepříjemnými pocity nesouvisejícími s nosní průchodností, které vedly k odmítnutí léčby.The objective and subjective effect of nasal patency- improving surgery (septoplasty/ turbinoplasty) on sleep- disturbed breathing is described in the study. The autors work with a group of 28 snorers with subjectively disturbed nasal patency including findings of anatomical deformity of nasal septum and/or inferior turbinate hypertrophy. The surgery has been performed if no respense to local nasal corticosteroids on subjective and objective findings has been observed. Comparing pre- and postoperative data of sleep examination (limited polygrafy) in all 28 patiens OSA has been proven. In 2 patients mild OSA, in 14 patients middle OSA and in 12 patients severe grade of OSA has been demonstrated. There was significant improvement both subjective and objective postoperative nasal patency observed, comparing post- and preoperative rhinomanometry data (p=0,0065). However, even of improved nasal patency, no significant change in AHI (Apnea/hypopnea index) has been noticed instead (p=0,0792). In the study, autors found significant improvement of analog self- monitoring scale of snoring 1-10 but on the other side, they didn´t find any significant improvement of daytime sleepiness (Epworth sleepiness scale). The autors conclude that septoplasty/turbinoplasty should not be considered as a OSA treatement metod, because no significant reduction of sleep respiratory disturbances has been observed. Disturbed nasal patency (with objective finding of nasal airways obstacles) should be considered as a main reason for surgical treatement. For patient, better subjective nasal breathing is a main goal of this procedure, but according to our findings, decrease of snoring could be expected as well. Another reason for nasal surgery is nasal CPAP therapy in indicated patiens- improved nasal patency should decrease pressure on CPAP mask and tolerance of therapy should be higher according to literature. This was unfortunately not proved in our study (from 8 indicated patiens, only 3 patients tolerated CPAP therapy). Low number of probands and another unpleasant fellings not concerning nasal patency could be reason for this.
