Syndrom HELLP je jednou z nejzávažnějších komplikací těhotenství. Jeho predikce je zatím nemožná a diagnóza bývá stanovena pozdě až na základě klinických příznaků. Vzhledem k závažnosti HELLP syndromu je jednou z priorit moderní perinatologie snaha o pochopení patofyziologických mechanismů spojených s preeklampsií a HELLP syndromem se zaměřením se na nalezení screeningových testů k časné identifikaci žen s rizikem rozvoje těchto komplikací. HELLP syndrom i preeklampsie jsou spojeny s výraznou aktivací endotelu a endotel je tudíž oblastí, kam se upíná primární výzkum. Disertační práce je dělená do tří částí, první část shrnuje současné znalosti o HELLP syndromu, v druhé části je uveden přehled současných znalostí o endotelu a třetí částí je vlastní výzkumná práce. Ta je zaměřená na sledování hladin markerů aktivace endotelu, jejich vývoj v jednotlivých trimestrech gravidity a vzájemné statistické porovnání vzorků od pacientek s fyziologickou graviditou, graviditou komplikovanou preeklampsií a graviditou komplikovanou HELLP syndromem.HELLP syndrome is one of the most serious complications of pregnancy. There are no useful tools for it´s prediction and the diagnosis is usually done based on the clinical criteria and usually very late. Therefore it is one of the priorities of modern perinatal medicine to understand pathophysiology of HELLP syndrom in order to find useful screening tests to help identify women at risk. HELLP syndrome is associated with endothelial activation, which is also area where the research is primarily focused. The thesis is divided into following parts ? a summary of the current knowledge about HELLP syndrome and it´s ethiology, a summary of the current knowledge about endothelium and the last part that contains the own study with results. The aim of the study was to determine markers of endothelial activation in order to diagnose endothelial damage during normal pregnancies, pregnancies complicated by preeclampsia and HELLP syndrome.
