Zavedení regulačních poplatků je důležitou součástí reformy zdravotnictví, resp. reformy veřejných financí České republiky. Touto reformou se s účinností od 1. ledna 2008 zavedly v systému českého zdravotnictví tzv. regulační poplatky, které stanovují definovaným skupinám pacientům hradit v rámci čerpání zdravotní péče specifický poplatek, jehož hlavním smyslem je regulovat nadužívání zdravotní péče. Regulační poplatek je hrazen v souvislosti s klinickým vyšetřením u lékaře, s hospitalizací v lůžkovém zdravotnickém zařízení, s návštěvou pohotovostní služby a s vyzvednutím léku na lékařský předpis. Předmětem diskuse je implementace regulačních poplatků v prvních dvou letech po jejich zavedení, především z ekonomických a právních hledisek. Na základě získaných dat jsou analyzovány dopady zavedení regulačních poplatků na jednotlivé skupiny subjektů v systému poskytování zdravotní péče, kterými jsou především poskytovatelé zdravotní péče, zdravotní pojišťovny a pacienti. Ze získaných poznatků je zřejmé, že poplatky alespoň částečně plní svou regulační funkci a jejich zavedení je správným, byť parciálním krokem v rámci reformy zdravotnictví v České republice.The implementation of regulatory fees is an important part of health care reform or, otherwise put, of public finance reform in the Czech Republic. This reform, through which regulatory fees have been implemented since January 1st, 2008, stipulates that the use of health care services by defined groups of patients requires them to pay a specific fee whose major significance is to regulate the abuse of health services. The regulatory fee is paid in the contexts of clinical examinations with a doctor, hospital admissions, and visits to an emergency medical ward as well as on the acquisition of prescription drugs. The initial two year period after the introduction of the regulatory fees are discussed herein mainly from economic and legal points of views. The impacts of the introduction of regulatory fees on the related individual subject groups of the health care system, namely health care providers, health insurance companies and patients are analyzed based on gathered data. From the knowledge acquired it is clear that the introduced fees at least partially fulfil their regulatory function and that their implementation is correct, albeit a partial step toward health care reform in the Czech Republic.
