Duchennova svalová dystrofie je progresivní onemocnění postihující především příčně pruhovanou svalovinu, tedy i svaly dýchací. Postižení dýchacích svalů je provázeno dechovou nedostatečností a bývá nejčastější příčinou úmrtí pacientů s DMD. Jedna část klinické léčby DMD je experimentálního charakteru (genová a buněčná terapie), druhá je pak zaměřena na řešení zmírnění progrese onemocnění. Názory na účinnost kinezioterapeutických přístupů se liší. Nejednoznačnost nad otázkou míry zatížení oxidačním stresem a tím destrukcí svalových buněk a schopnosti adaptace organismu na zátěž stále zůstává. Péče o respirační systém je nesporně významný jak z hlediska prevence komplikací zahleněním a alveolární nevzdušnosti, tak z pohledu potencování svalové dysfunkce v důsledku kyslíkové dependence. Z tohoto důvodu je respirační fyzioterapie velmi důležitou součástí léčby DMD. Důkazné objasnění molekulárně biochemických pochodů svalové adaptace na zátěž formou respiračního tréninku u dystrofických nemocných není stále uspokojivě dořešeno.Duchennen's muscular dystrophia is a progressive disease concerning mainly striated muscles, so also the respiratory muscles. Such affection of the respiratory muscles is usually connected to respiratory insufficiency which is the most often cause of death of the patients with DMD. One part of the clinical treatment of DMD is of an experimental character (genic and cellulite therapy) and the other part is aimed at reduction of progression of the disease. There are various opinions at the effectivity of the kinesio-therapeutical approaches. The rate of oxidative stress that causes the destruction of muscular cells and the ability of the organism to adapt is still questionable. A care of the respiratory system is without disputate very important either because of the prevention of complications with the expectoration and alveolar non-airiness or from the point of view of the potential muscular dysfunction caused by the oxygen dependence. That's why is the respiratory physiotherapy a very important part of the DMD treatment. A real and exact clarification of the molecular-biochemical processes of the muscular adaptation on the respiratory training stress by dystrophic patients is still missing.
