Patogeneze kastračně rezistentního karcinomu prostaty, který se vyvíjí po androgenové deprivační terapii, je ovlivňována nejen genetickými, ale i epigenetickými faktory, z nichž významné místo zaujímají metylace DNA a posttranslační modifikace histonů. Pro progresi onemocnění je důležitý status a funkce androgenového receptoru, přičemž u CRPC dochází k omezeným možnostem jeho regulace v důsledku na androgenech nezávislé proliferace nádorových buněk. Činidla s potenciálem epigenetického účinku mohou vést k obnovení exprese inaktivovaného androgenového receptoru u pacientů s CRPC a případně zmírnit proliferační aktivitu nádorových buněk. Cílem této studie bylo analyzovat účinky dvou epigenetických činidel, inhibitoru DNA metyltransferáz (5-Aza-2´-deoxycytidin) a inhibitoru histonových deacetyláz (butyrát sodný) na potenciální změny v expresi androgenového receptoru a viabilitu prostatických nádorových buněk, které reprezentují CRPC stádium a jsou bez exprese androgenového receptoru. Zjistili jsme, že kombinovaný účinek obou použitých činidel vedl u ovlivněných prostatických nádorových buněk k re-expresi androgenového receptoru spolu s demetylačními změnami promotoru androgenového receptoru, zvýšené acetylaci histonu H4 a indukci apoptózy. V důsledku omezené účinnosti epigenetické monoterapie a komplexnosti epigenetických modifikací v nádorech jde mnoho recentních studií cestou kombinované terapie, která zahrnuje aplikaci více epigenetických inhibitorů a rovněž kombinace epigenetických inhibitorů s chemoterapií, hormonální terapií a/nebo inhibitory imunitních kontrolních bodů. Proto může být studium účinků epigenetických inhibitorů u kastračně rezistentního karcinomu prostaty přínosné pro pochopení dalších mechanismů vedoucích k progresi tohoto závažného nádorového onemocnění.The pathogenesis of castration-resistant prostate cancer, which manifests itself after androgen deprivation therapy, is influenced not only by genetic but also by epigenetic factors, of which DNA methylation and post-translational modifications of histones play important roles. Androgen receptor status and function are important for disease progression, and CRPC has limited regulatory potential due to androgen-independent proliferation of tumour cells. Agents with the potential for epigenetic effect may restore inactivated androgen receptor expression in patients with CRPC and possibly attenuate tumour cell proliferative activity. The aim of this study was to analyse the effects of two epigenetic agents. A DNA methyltransferase inhibitor (5-Aza-2´-deoxycytidine) and a histone deacetylase inhibitor (sodium butyrate), on potential changes in androgen receptor expression, viability of prostate tumour cells without androgen receptor expression, and represent the CRPC stage. We found that the combined effect of both agents used, in addition to the affected prostate tumour cells on androgen receptor re-expression with demethylation changes of the androgen receptor promoter, together with histone H4 acetylation and induction of apoptosis. Due to effective epigenetic monotherapy and the complexity of epigenetic modifications in tumours, many recent studies have taken the path of combination therapy. This involves the administration of multiple epigenetic inhibitors, as well as a combination of epigenetic inhibitors with chemotherapy, hormonal therapy and/or immune checkpoint inhibitors. Therefore, studying the effects of epigenetic inhibitors in castration-resistant prostate cancer may be helpful in understanding other mechanisms leading to the progression of this serious cancer.
