Diplomová práce je zaměřena na diagnostiku poruch polykání u dětí metodou videofluoroskopie (VFSS) a její přínos ve srovnání s flexibilním endoskopickým vyšetřením polykání (FEES). Teoretická část popisuje obě zmíněné metody, polykací proces včetně odlišností u dětí a dospělých, definuje a charakterizuje dysfagii a její terapii. Cílem práce bylo srovnání diagnostické výtěžnosti VFSS a FEES a zhodnocení VFSS provedených ve FN Brno, pracoviště Dětské nemocnice. Kolektivizace dat probíhala retrospektivní formou metodou analýzy nálezů. Ve FN Brno bylo v období 2019-2022 125 pacientů dětského věku indikováno k vyšetření VFSS. Nejvíce vyšetřených bylo mladších než jeden rok. Nejčastěji se jednalo o pacienty, kteří jsou sledováni pro primární onemocnění, s největším zastoupením dětí s vrozeným/získaným neurologickým postižením. Porucha v průběhu transportu sousta byla metodou VFSS detekována u 75,2 % vyšetřených, nejčastěji byla zjištěna v průběhu orální a faryngeální fáze. Do komparace VFSS s FEES bylo zahrnuto 21 pacientů. Metody byly srovnávány ve 4 funkčních parametrech - leaking, rezidua v orofaryngu, insuficience velofaryngeálního uzávěru (VFU) a průchod sousta do dýchacích cest (DC). Za použití Cohenova koeficientu bylo zjištěno, že mezi metodami není téměř žádná shoda až průměrná. Oproti FEES je VFSS lepší volbou v hodnocení insuficience VFU a rozsahu průchodu bolu do DC. V hodnocení leakingu a reziduí je FEES vhodnou alternativou. Statistickým testem byl zjištěn signifikantní rozdíl ve výsledcích hodnocení rozsahu průniku sousta do DC, v ostatních případech nebyl rozdíl statisticky významný.The diploma thesis focuses on a diagnostics of dysphagia by a videofluoroscopic swallowing study (VFSS) and comparison between the VFSS and an endoscopic evaluation of swallowing (FEES). The work is divided into a theoretical and a practical part. The theoretical part describes both methods, swallowing and its differences between pediatrics and adults, defines and characterizes dysphagia and the therapy. The aim of the practical part is the evaluation of all VFSS performed at FN Brno and comparison between VFSS and FEES. For the practical part the data was collected retrospectively by analyzing the results of the examinations. At the FN Brno there were 125 pediatric patients reffered for VFSS in the period 2019-2022. Most of them were younger than one year. In most cases, the patients were treated for some disease, with the highest proportion of children with neurological disabilities. A dysphagia disorder was detected in 75.2 % of the examined, most often it was detected during the oral and pharyngeal phase. 21 patients were included in the comparison of VFSS with FEES. The methods were compared in 4 functional parameters - a velopharyngeal insufficiency/inadequancy (VPI), the residue in the oropharynx, early spillover and passage of food/liquid to a respiratory tract. Cohen's coefficient was calculated, it was found there is almost no agreement to average between the methods. VFSS against FEES shows better detection of VPI and evaluation passage into the respiratory tract. In early spillover and the residue evaluation, FEES is a suitable alternative for evaluating residue and early spillover based on average compliance. A statistical test revealed a significant difference in the results of evaluation of passage to the respiratory track, in the other cases the difference was not statistically significant.
