Poruchy dýchání ve spánku jsou významnou příčinou morbidity a snížené kvality života pacientů. Mezi nejvýznamnější a nejčastější patří syndrom obstrukční spánkové apnoe a hypoventilační syndrom při obezitě. Přetlaková ventilace je zde dosud zlatým standardem v terapii. V této práci jsou shrnuty aktuální poznatky z výzkumu komplikací poruch dýchání ve spánku a terapie přetlakovou ventilací v jejich prevenci. V praktické části se zaměřujeme na distribuční šíři erytrocytů (RDW) a střední destičkový objem (MPV) u pacientů s obstrukční spánkovou apnoí. Prokázali jsme slabou, ale statisticky signifikantní korelaci RDW s parametry tíže obstrukční spánkové apnoe (OSA). MPV a RDW mohou být markery zvýšeného kardiovaskulárního rizika u pacientů s OSA. V dalším souboru pacientů s OSA jsme prokázali korelaci hladiny adipocite fatty-acid binding protein (A-FABP) a tloušťky epikardiálního tuku. Touto korelací doplňujeme další dílek k poznání příčin zvýšené kardiovaskulární morbidity a mortality u pacientů s OSA. Při desetiletém sledování compliance pacientů s terapií přetlakovou ventilací u pacientů s OSA jsme prokázali, že nejvýznamnější prediktor dobré spolupráce s pacientem je tíže OSA a jeho vstupní symptomatika. Dobrá compliance (více než 4 hod CPAP za noc) byla zaznamenána u přibližně 57 % pacientů a tato úroveň compliance se po roce sledování již dále nezměnila. Na závěr praktické části jsme vypracovali analýzu vlivu pandemie COVID-19 na činnost spánkových laboratoří v České a Slovenské republice. Práce analyzovala kolaterální škody jarních restrikcí pomocí dotazníkového průzkumu z akreditovaných spánkových laboratoří. Odhadujeme, že jen během období březen-květen 2020 nebylo v analyzovaném regionu provedeno asi 1000 diagnostických vyšetření a 1900 titrací terapie přetlakovou ventilací.Sleep disordered breathing (SDB) is an important factor contributing to a higher morbidity, and a decreased quality of life in patients. The most important and frequent types of SDB are obstructive sleep apnoea (OSA) syndrome, and hypoventilation syndrome in obesity; positive airway pressure therapy remains the gold standard for care. This thesis summarizes the current knowledge about the complications of SDB, and the importance of positive airway pressure therapy in their prevention. In the practical section we focus on the red cell distribution width (RDW), and mean platelet volume (MPV) in patients with OSA. A weak, but statistically significant correlation, between RDW and parameters of OSA severity were found; MPV and RDW are possible markers of higher cardiovascular risk in patients with OSA. In the next study group of patients with OSA, we found a significant correlation between adipocite fatty-acid binding protein (A-FABP) levels, and epicardial fat thickness. We consider this finding a possible contribution to strengthen the knowledge regarding the link between OSA and cardiovascular morbidity and mortality. In the 10 year follow-up of patients with OSA treated with continuous positive airway pressure (CPAP) therapy, we found that the most important predictor of good compliance to therapy was the severity of the disorder. Sufficient compliance (defined by an average CPAP use of 4 hours or more per night) was present in 57 % of patients. In the final chapter of the practical section, we analysed the impact of the Covid-19 pandemic on sleep medicine in the Czech and Slovak Republic. The study utilised questionnaire responses, to determine possible collateral damages caused by spring restrictions. We estimate that in the period of March-May 2020, approximately 1000 sleep studies and 1900 titrations of the therapy were missed.
