Úvod: Diplomová práce je zaměřena na zjištění přínosu skiagrafického vyšetření lebky z různých indikací. Výzkum bude probíhat jako retrospektivní studie na pracovišti radiologie Nemocnice Rokycany Cíl: Hlavním cílem práce je statistické šetření četnosti indikace vyšetření rentgenového vyšetření lebky a zjištění jeho přínosu k léčebnému postupu. Vedlejšími cíli je zjistit poměr pozitivních nálezů, souvislost mezi věkem pacienta a počtem indikovaných vyšetření a zjištění dávkového zatížení a rizika způsobená pacientovi. Metoda: Ze souboru pacientů, kteří podstoupili vyšetření lebky v průběhu roku 2021, byl vybrán výběrový statistický soubor, na němž proběhla analýza důvodu indikace, správnosti indikace, korelace s věkem, návaznosti dalších zobrazovacích metod a dávkového zatížení. Výsledky: Bylo zjištěno, že v případě skiagrafického vyšetření lebky se jedná převážně o nevhodně indikované případy, počet pozitivních nálezů byl minimální. Nepodařilo se potvrdit souvislost mezi věkem pacienta a pravděpodobností indikace skiagrafie lebky. Byl vypočítán odhad efektivní dávky vypovídající o radiačním zatížení pacienta a bylo stanoveno riziko způsobené zářením pomocí veličiny LLE. Závěr: Skiagrafické vyšetření lebky jako metoda první volby u pacienta s kraniocerebrálním traumatem není vhodné. Pozitivita nálezů je minimální, čímž nedochází k uplatnění principu odůvodnění tohoto vyšetření.Introduction: The diploma thesis focused on finding out the benefits of plain skiagraphic examination of the skull from various indications. The research will take place as a retrospective study at the radiology department of the Rokycany Hospital Objective: Main objective of the course in terms of learning outcomes and competences. The secondary objectives are to determine the ratio of positive findings, the relationship between the patient's age and the number of indicated examinations, and to determine the dose burden and risks caused to the patient. Method: From the group of patients who underwent skull examinations during 2021, a sample statistical file was selected, on which the reason for the indication, the accuracy of the indication, the correlation with age, the continuity of other imaging methods and the dose load were analyzed. Results: It was found that in the case of skiagraphic examination of the skull, these are mostly inappropriately indicated cases, the number of positive findings was minimal. The relationship between the patient's age and the probability of indicating skull skiagraphy could not be confirmed. An estimate of the effective dose indicating the radiation exposure of the patient was calculated and the risk caused by radiation was determined using the LLE value. Conclusion: Skiagraphic examination of the skull as the method of first choice in patients with craniocerebral trauma is not appropriate. The positivity of the findings is minimal, which does not apply the principle of justification of this examination.
