Úvod: Robotické rehabilitační systémy jsou využívány u neurologických pacientů ke zmírnění motorických poruch horní končetiny a zlepšení výkonu pacienta ve funkčních aktivitách. Cíl: Vyhodnotit efekt roboticky asistované rehabilitace u pacientů se spastickou parézou horní končetiny po cévní mozkové příhodě (CMP) na míru spasticity, funkci horní končetiny a soběstačnost v rámci běžných každodenních činností. Metodika: Výzkumu se zúčastnilo celkem 67 probandů se spastickou parézou horní končetiny v chronickém stádiu po CMP. Probandi byli náhodně rozděleni do kontrolní a experimentální skupiny. Obě skupiny podstoupily klasický rehabilitační program doplněný 5 hodinami roboticky asistované rehabilitace (RAR) v experimentální skupině a 1 hodinou RAR týdně v kontrolní skupině. Probandi absolvovali dvě měření: vstupní vyšetření proběhlo před zahájením terapie a výstupní po měsíční terapii. Změna spasticity m. brachialis a m. brachioradialis byla hodnocena dle Modifikované Ashworthovy škály (MAS) a Tardieu škály (TS). Pro zhodnocení funkce HK byl využit Modifikovaný Frenchayský test paže (MFS), soběstačnost v ADL byla vyhodnocena na základě Barthel indexu (BI). Výsledky: Při porovnání výsledků obou skupin, bylo zjištěno, že došlo v experimentální skupině ke statisticky signifikantnímu zlepšení (p <0,05) soběstačnosti v rámci ADL, funkce horní končetiny i stupně spasticity měřené dle Tardieu škály, což potvrdilo efektivitu robotické terapie. Jediným měřeným parametrem, kde nebyly pozorovány statisticky signifikantní změny je míra spasticity hodnocená dle MAS. Závěr: Roboticky asistovaná rehabilitace má pozitivní vliv na snížení spasticity, zlepšení funkce horní končetiny i zvýšení soběstačnosti v ADL a je možné na ni pohlížet jako na efektivní doplněk klasické neurorehabilitace.Introduction: Robotic rehabilitation systems are being used with neurological patients to alleviate their upper limb motor disorders and to improve the patient's performance in functional activities. Aim: Evaluation of the effect of robot-assisted rehabilitation in stroke patients with upper limb paresis on the level of spasticity, upper limb function and self-sufficiency in activities of daily living. Methods: Overall, a total of 67 patients with spastic upper limb paresis in the chronic stage after stroke participated in the research. Patients were randomly divided into control and experimental group. Both groups completed a conventional rehabilitation program supplemented by 5 hours of robot-assisted rehabilitation (RAR) in the experimental group and 1 hour of RAR per week in the control group. The patients underwent two measurements: the initial examination took place before the start of therapy and the final examination took place after one month of therapy. The change in spasticity of the brachialis muscle and brachioradialis muscle was assessed by the Modified Ashworth Scale (MAS) and the Tardieu Scale (TS). The Modified Frenchay arm test (MFS) was used to evaluate upper limb function and self-sufficiency in ADL was evaluated by the Barthel index (BI). Results: When comparing the results of both groups, there was a statistically significant improvement (p < 0.05) in experimental group within: self-sufficiency performing ADL, upper limb function and the degree of spasticity measured by the Tardieu scale, which confirmed the effectiveness of the robotic therapy. The only measured parameter, where no statistically significant changes were observed. is the degree of spasticity evaluated by the Modified Ashworth Scale. Conclusion: Robot-assisted rehabilitation has a positive effect on reducing spasticity, improving upper limb function and increasing self-sufficiency in ADL and can be seen as an effective complement to the conventional neurorehabilitation.
