Práce studovala široké využití stanovení neopterinu u nemocných léčených cílenou léčbou nebo imunoterapií. V teoretické části jsou shrnuty poznatky o maligním melanomu, imunoterapii a využití neopterinu v klinické medicíně. V praktické části byla na základě výchozích kasuistik provedena analýza neopterinu v séru u nemocných s imunitně podmíněnými nežádoucími účinky. Výsledky ukazují, že stanovení neopterinu v séru může být vhodné k monitorování toxicity imunoterapie. Ve skupině nemocných s maligním melanomem léčených ipilimumabem byl neopterin v séru nezávislým prognostickým faktorem. V další části práce byla popsána úloha neopterinu v diferenciální diagnostice horečky u pacientky léčené kombinovanou cílenou terapií.The thesis examines wide utilization of the neopterin in patients treated with targeted therapy or immunotherapy. In the teoretical part are summarised findings about melanoma, immunotherapy and neopterin in clinical practice. In the original part of the thesis based on initial cases reports is performed analysis of serum neopterin in patients with immune-related adverse events. These results show that the determination of neopterin in serum could be suitable for the monitoring toxicity of the immunotherapy. Neopterin is an independent prognostic factor in the group of patients with metastatic melanoma treated by ipilimumab. In the next part of the thesis is described the role of neopterin in the differential diagnosis of pyrexia in a patient treated with combined targeted therapy.
