IgA nefropatie je nejčastější primární glomerulonefritida na světě. Je charakteristická tvorbou galaktosově deficientního IgA. To rozpoznávají specifické autoprotilátky IgG a spolu pak tvoří cirkulující imunitní komplexy. Ty se následně ukládají v glomerulech a tam spouští mesangiální proliferaci, expanzi matrix a tvorbu zánětlivých faktorů. V současné době je léčba založena na obecném zpomalení úbytku ledvinových funkcí a je podobná jako u ostatních ledvinových onemocnění. Pro budoucí léčbu se zde nachází možnosti kauzální terapie, které mohou být zaměřeny na inhibici buněk tvořících gd IgA1, snížení molekulové hmotnosti imunitních komplexů, inhibice proliferace mesangiálních buněk či štěpení již uložených imunitních komplexů v mesangiích. V této diplomové práci bylo vytvořeno polymerní degalaktosylované IgA1 a také 1123 IgG autoprotilátka. Pomocí těchto byly tvořeny imunitní komplexy, které pak byly detekovány metodou ELISA a jejich afinita měřena metodou SPR. Bylo detekováno zvýšení vaznosti oproti kontrolním vzorkům. Následně byly detekovány gd IgA1 v sérech pacientů, stejně tak autoprotilátky v sérech pacientů a afinita mezi gd IgA1 ve vybraném séru pacienta a vytvořenou autoprotilátkou. Data budou využita při dalším výzkumu malých rekombinantních inhibitorů imunitních komplexů.IgA nephropathy is the most common primary glomerulonephritis worldwide. It's characterized by the formation of galactose deficient IgA, which is recognized by specific IgG antibodies and together they form circulating immune complexes. These are deposited in the glomeruli, where they trigger mesangial proliferation, matrix expansion and the formation of inflammatory factors. Available treatment is based on a slowing of the loss of kidney function and it's similar as the treatment of other kidney diseases. For future treatment, there are some options of causal therapy, that can be aimed at inactivation of the cells producing galactose deficient IgA1, reducing the size of immune complexes, stopping of mesangial cells activation or cleavage of immune complexes already deposited in mesangium. This diploma thesis was focused on production of degalactosylated polymeric IgA1, production of 1123 IgG autoantibody. De novo formation of immune complexes of dg IgA1 and the 1123 IgG autoantibody, which were detected by ELISA and their affinity measured by SPR. An increase of binding compared to control samples was detected. Finally, gd IgA1 in the sera was detected, as well as autoantibodies in the patient's sera and the affinity between the gd IgA1 in the selected patient's serum and recognized by the autoantibody. The data will be used in further research on small recombinant inhibitors of immune complexes.
