Úvod: Při představě a provedení pohybu se aktivují do značné míry identické korové oblasti. Chronická bolest vede k maladaptivní neuroplasticitě senzomotorických kortikálních oblastí podílejících se na vnímání a řízení pohybu. Terapeutický koncept stupňované představy pohybu (graded motor imagery - GMI), využívající imaginaci potencuje reparační neuroplastické změny postižených oblastí s cílem snižovat chronickou bolest. Cíl: Zhodnotit efektivitu GMI jako terapie chronické bolesti u pacientů s bolestivým ramenem. Dílčími cíli bylo posouzení efektu terapie na rozsah pohybu a strach z bolesti. Metodika: Terapie se zúčastnilo 8 probandů s bolestí ramenního pletence trvající déle než 6 měsíců. U všech probandů byla vyšetřena bolest pomocí Vizuální analogvé škály (VAS) a Dotazníku interference bolesti s denními aktivitami (DIBDA), strach z bolesti pomocí Pain catastrophizing scale (PCS) a rozsah pohybu v rameni před a po terapii. GMI se skládala ze tří fází terapie: určování laterality končetin, představa pohybu bolestivým segmentem, zrcadlová terapie formou sledování videosekvence. Výsledky: Po terapii došlo ke statisticky významnému zlepšení pohybu do ventrální flexe a snížení katastrofizace bolesti (p < 0,05). Došlo také k mírnému snížení bolesti na škále VAS i DIBDA a zvýšení rozsahu do zevní rotace a abdukce, nejednalo se však o statisticky významné zlepšení. Závěr: GMI v naší pilotní studii vedlo ke snížení katastrofizace bolesti a ke zvýšení rozsahu pohybu v ramenním pletenci u pacientů s chronickou bolestí ramene. Vliv konceptu GMI na vnímání bolesti nebyl signifikantní. Pro verifikaci potenciálu GMI snižovat chronickou bolest je žádoucí rozšířit soubor pacientů zahrnutých do studie.Introduction: During imaging and executig certain movement, there are active largely identical cortical areas. However, chronic pain contributes to maladaptive neuroplasticity of sensorymotor cortical areas ivolved in perception and control of the movement. Therapeutic concept of Graded motor imagery (GMI) using imagination potentiates repair neuroplastic changes of affected areas in order to reduce chronic pain. Aim: To evaluate the effectiveness of graded motor imagery therapy of chronic pain for patients with shoulder pain. Sub-objectives are to evaluate the effectiveness of GMI therapy to the range of motion and fear of movement. Methods: 8 probands with chronic shoulder pain lasting more than 6 months participated in the therapy. All probands were examined before and after therapy for pain using the Visual Analogue Scale (VAS) and the Daily Activities and Pain Interference Questionnaire (DIBDA). Pain catastrophisation was assessed by the Pain Catastrophizing Scale (PCS), and shoulder active range of motion was measured. GMI consisted of 3 phases: laterality training, motor imagery of painful body part and mirror therapy in the form of watching a videosequence. Results: Significant improvement of ventral flexion movement and significant reduction of pain catastrophisation were measured after the therapy (p < 0.05). Mild pain reduction on VAS and DIBDA scales, and mild increase in external rotation and abduction movements were measured, but these results were not significant. Conclusion: GMI in our pilot study led to a reduction of pain catastrophizing and an increase in range of motion of shoulder griddle in patients with chronic shoulder pain. The influence of GMI concept on pain perception was not significant. To verify the potential of GMI to reduce chronic pain it is desirable to extend the file of patients included in the study.
