Úvod: Chodící pás se u pacientů po cévní mozkové příhodě (CMP) využívá v rámci rehabilitace k dosažení největší možné samostatnosti chůze a celkové soběstačnosti postiženého jedince. Cíl: Zhodnocení parametrů chůze pomocí chodícího pásu Zebris u pacientů po cévní mozkové příhodě (CMP). Metodika: Studie se zúčastnilo celkem 8 probandů (7 mužů a 1 žena) po první atace ischemické cévní mozkové příhody (ICMP). Průměrný věk probandů byl 66 ? 13,4 let, průměrná výška byla 172 ? 6,8 cm a průměrná hmotnost byla 78 ? 3,9 kg. Minimální hranice pro zařazení do studie byl výsledek testu Functional Ambulation Category stupeň 4. První měření probíhalo průměrně 11,6 ? 7,7 dní po atace a průměrná doba mezi 1. a 2. měřením byla 10 ? 4,1 dne. Každý pacient podstoupil 2 měření, a to na začátku a po ukončení rehabilitační péče na lůžkovém oddělení FNOL, kde byla hodnocena minutová chůze na páse individuálně zvolenou rychlostí a 2 chůzové testy na zemi (10MWT a TUG test). Hodnoceny byly parametry z reportu systému Zebris a výsledky chůzových testů. Pro statistické zpracování výsledků byl využit Wilcoxonův test. Výsledky: Při 2. měření byly signifikantní výsledky shledány pro fáze krokového cyklu, konkrétně pro parametry stojná fáze paretické (p=0,017) i neparetické DK (p=0,012), stádium zatěžování pro neparetickou DK (p=0,012), mezistoj pro paretickou (p=0,017) i neparetickou DK (p=0,025), předšvihová fáze pro paretickou DK (p=0,012), švihová fáze pro paretickou (p=0,017) i neparetickou DK (p=0,012) a pro součet fází dvojí opory (p=0,012). Z maximálního tlakového působení byly signifikantní výsledky pro předonoží paretické (p=0,05) i neparetické DK (p=0,017) a pro středonoží neparetické DK (p=0,012). Z procentuálních parametrů se jednalo o výsledky pro zadonoží paretické (p=0,012) i neparetické DK (p=0,017). Z chůzových testů byly statisticky významné výsledky pro pohodlné tempo chůze (p=0,025) i pro maximální možné tempo chůze (p=0,035) a v rychlosti v TUG testu (p=0,011. Závěr: Zařazení chodícího pásu do rehabilitační léčby pacientů po CMP má pozitivní vliv na parametry jejich chůze, přičemž toto zlepšení se promítá i do chůze po zemi.Introduction: In patients after stroke the treadmill is used in rehabilitation to achieve the biggest possible walking independence and self-sufficiency in activities of daily living. Objective: Evaluation of the effect of therapy using the Zebris treadmill on gait parameters in patients after stroke. Methodology: In total 8 probands (7 men and 1 woman) after the first attack of ischemic stroke were part of this study. The mean age of the probands was 66 ? 13.4 years, the mean height was 172 ? 6.8 cm and the mean weight was 78 ? 3.9 kg. The minimum limit to take part in the study was the Functional Ambulation Category grade 4. The first measurement was averaged 11.6 ? 7.7 days after attack and the mean time between the 1st and 2nd measurements was 10 ? 4.1 days. Each patient underwent 2 measurements, at the beginning and after the end of rehabilitation treatment in the inpatient department of University Hospital Olomouc. One-minute long walk on the belt at individually selected speed and 2 walk tests on the ground (10MWT and TUG test) were assessed. Parameters from the Zebris system report and the results of walk tests were evaluated. The Wilcoxon test was used for statistics. Results: Within 2nd measurement, significant results were found for some phases of the gait cycle, specifically for the parameters stance phase of paretic leg (p = 0.017) and non-paretic leg (p = 0.012), load response for non-paretic leg (p = 0.012), midstance for paretic leg (p = 0.017) and non-paretic leg (p = 0.025), pre-swing phase for paretic leg (p = 0.012), swing phase for paretic leg (p = 0.017) and non-paretic leg (p = 0.012) and for the double stance phase (p = 0.012) . From the maximum pressure the significant results were found for both paretic (p = 0.05) and non-paretic leg (p = 0.017) for forefoot and for non-paretic leg (p = 0.012) for midfoot. The percentage parameters of time maximum force showed significant results for both paretic (p = 0.012) and non-paretic leg (p = 0.017). Statistically significant results for a comfortable walking pace (p = 0.025) as well as for the maximum possible walking pace (p = 0.035) and for speed in the TUG test (p = 0.011) were shown by walk tests. Conclusion: The addition of a treadmill Zebris to the rehabilitation treatment of patients after stroke has a positive effect on the gait parameters and, moreover, this improvement is also reflected in walking on the ground.
