Parkinsonova nemoc je spojena s mnoha nemotorickými příznaky, které mohou předcházet příznakům motorickým. Mezi tyto nemotorické příznaky patří autonomní dysfunkce, které se často vyskytují již v rané fázi onemocnění a značně snižují kvalitu života pacienta. Cílem diplomové práce bylo zhodnotit míru autonomních dysfunkcí a progresi jednotlivých autonomních poruch u pacientů s Parkinsonovou nemocí v závislosti na stadiu onemocnění dle Hoehnové a Yahra. Pro zhodnocení míry autonomních dysfunkcí byl použit dotazník SCOPA-AUT, který vykazuje vysokou míru validity a reliability. Práce zahrnuje také porovnání autonomních funkcí s motorickými funkcemi pacienta hodnocenými pomocí škály Parkinson Activity Scale. Výzkumný soubor byl složen z 31 probandů s Parkinsonovou nemocí, 17 mužů průměrného věku 70,5 +- 4,8 let a 14 žen průměrného věku 70,6 +- 7,2 let, ve stadiu onemocnění 1-4 dle Hoehnové a Yahra. Výsledky diplomové práce prokázaly na hladině statistické významnosti (p<0,05) pozitivní korelaci mezi celkovým skóre dotazníku SCOPA-AUT, autonomními položkami dotazníku a stadiem onemocnění dle Hoehnové a Yahra. Urinární a sexuální dysfunkce statisticky významnou korelaci neprokázaly. Dále byla zjištěna pozitivní korelace mezi celkovým skóre dotazníku SCOPA-AUT a jednotlivými autonomními položkami kromě sexuálních funkcí. Délka trvání nemoci ani věk probandů na výsledky dotazníku SCOPA-AUT neměly vliv. Gastrointestinální a urinární dysfunkce se s vyšší četností vyskytovaly již v prvním stadiu Parkinsonovy nemoci. Mezi výsledky dotazníku SCOPA-AUT a hodnocením motorických funkcí pomocí škály Parkinson Activity Scale nebyla nalezena statisticky významná korelace.Parkinson's disease is associated with many non-motor symptoms which may be followed by motor symptoms. These non-motor symptoms include autonomic dysfunction that often occur early in the disease and greatly reduce the patient´s quality of life. The aim of the thesis has been to evaluate the degree of autonomous dysfunction and progression of individual autonomic disorders in patients with Parkinson's disease depending on the stage of the disease according to Hoehn and Yahr. The SCOPA-AUT questionnaire, which shows a high degree of validity and reliability, was used to assess the degree of autonomous dysfunction. The work also includes a comparison of autonomous functions with motor skills of the patient evaluated by the means of the Parkinson Activity Scale. The research group consisted of 31 probands with Parkinson's disease, 17 men with the average age of 70.5 +- 4.8 years and 14 women with the average age of 70.6 +- 7.2 years, at disease stage 1-4 according to Hoehn and Yahr. The results of the thesis showed a positive correlation between the overall score of the SCOPA-AUT, the autonomous items of the questionnaire and the stage of the disease according to Hoehn and Yahrat the level of statistical significance (p<0.05). Neither urinary nor sexual dysfunction showed statistically significant correlation. In addition, a positive correlation was found between the overall SCOPA-AUT score and individual autonomous items except for sexual functions. Duration of illness or age of probands had no influence on the results of the SCOPA-AUT. Gastrointestinal and urinary dysfunction already ocurred with higher frequency in the first stage of Parkinson's disease. No statistically significant correlation was found between the results of the SCOPA-AUT and the evaluation of motor functions using the Parkinson Activity Scale.
