Mentální retardace (MR) je závažné onemocnění, které postihuje celosvětově 1-3% dětí. Geneticky podmíněná MR může být vyvolána změnami na mnoha úrovních celé genetické informace od chromozomálních aberací až po změny jediného genu. Celosvětově jsou popsány stovky genů asociovaných s touto nemocí, ale testování takového množství genů u pacientů s MR je nákladné, zdlouhavé a má nízkou efektivitu. V této práci byla provedena CNV analýza 59 pacientů s MR metodami SNP array a MLPA po vyloučení chromozomálních aberací a Syndromu fragilního X. Klinický význam nalezených CNV a anotace genů zasažených těmito variantami byly stanoveny pomocí genetických databází (ClinVar, DECIPHER, OMIM) pro účely masivně paralelního sekvenování (MPS). Celkem bylo anotováno 42 genů, které jsou asociovány s poškozením centrálního nervového systému, abnormálním vývojem corpus callosum, degenerací/špatnou funkcí neuronů v mozku a/nebo epilepsií, popř. abnormálním EEG. Tyto geny považujeme za významné z hlediska genetické diagnostiky mentální retardace, a proto navrhujeme jejich zařazení do panelů masivně paralelního sekvenování, které by umožnilo identifikovat přítomnost sekvenčních změn těchto genů. Důkladný výběr genů pro tyto účely by mohl zvýšit efektivitu a snížit finanční náklady celé genetické diagnostiky pacientů s mentální retardací.Intellectual disability (ID) is a serious disorder that affects 1-3 % of children worldwide. Genetic conditioned ID can be induced by changes at many levels of the entire genetic information, such as chromosomal aberrations or changes in single genes. Hundreds of genes were associated with this disorder, but the testing of so much genes in ID patients is expensive, time-consuming and low-efficiency. CNV analysis was prepared in 59 patients with ID by MLPA and SNP array. Chromosomal aberrations and Fragile X syndrome were excluded. The clinical significance of the found CNVs and the annotation of the affected genes was determined using genetic databases (ClinVar, DECIPHER, OMIM) for purposes of next generation sequencing (NGS). In total, 42 annotated genes that are associated with central nervous system damage, abnormal development of the corpus callosum, degeneration/malfunction of brain's neurons and/or epilepsy, alternatively abnormal EEG. We consider these genes to be important for genetic diagnosis of intellectual disability, and therefore we suggest their inclusion in next generation sequencing panels to identify the presence of sequence changes in these genes. A thorough selection of genes for these purposes could increase efficiency of testing and so does reduce the cost of the entire genetic diagnosis of intellectual disability patients.
