Cíl studie: Určení incidence specifických klinicky významných antierytrocytárních aloprotilátek u těhotných žen v I. trimestru těhotenství v olomouckém regionu. Předmět a metodika studie: V letech 2000 až 2015 bylo vyšetřeno celkem 54 657 těhotných žen na Transfuzním oddělení Fakultní nemocnice Olomouc. V prvním trimestru těhotenství byl u všech žen proveden screening nepravidelných tepelných antierytrocytárních protilátek, a dále pak i identifikace protilátkových nálezů. Výsledky: Klinicky významné antierytrocytární aloprotilátky byly detekovány u 1,7 % těhotných žen (899/54657). Nejčastější příčinou erytrocytární aloimunizace v těhotenství byl antigen "E" 6,3 (342/54657), následován antigeny "D" 4,1 (222/54657), "M" 1,8 (97/54657), "K" 1,3 (71/54657), "C" 1,2 (67/54657), "c" 0,9 (47/54657), "S" 0,4 (23/54657), "Jka" 0,2 (11/54657), "PP1pk" (Tja) 0,1 (4/54657) a "Fya" 0,1 (6/54657). Závěr: I přes provádění profylaxe anti-D imunoglobulinem u RhD negativních žen představuje RhD antigen druhou nejčastější, avšak klinicky nejvýznamnější příčinu mateřské aloimunizace. Klinický problém rovněž představují non-D antigeny z Rh systému, antigeny z Kell systému, méně často antigeny ze systémů MNS a Kidd.Aim of the Study: To determine the incidence of clinically significant anti-erythrocyte alloantibodies in pregnant women, which can cause severe Hemolytic Disease in the Fetus and Newborn. Subject and method: Between the years 2000-2015, a total of 54 657 pregnant women were examined at the Department of Transfusion Medicine at the University Hospital Olomouc. Screening for irregular anti-erythrocyte antibodies followed by identification of the alloantibody was performed in all women at the beginning of the pregnancy. Results: Clinically significant anti-erythrocyte antibodies were diagnosed in 1.7% pregnant women (899/54657). The most common cause of maternal alloimmunization was antigen "E" with an incidence of 6.3 (342/54657), followed by antigen "D" 4.1 (222/54657), "M"1.8 (97/54657), "K" 1.3 (71/54657), "C" 1.2 (67/54657), "c" 0.9 (47/54657), "S" 0.4 (23/54657), "Jka" 0.2 (11/54657), "PP1pk" (Tja) 0.1 (4/54657) and antigen "Fya" 0.1 (6/54657). Conslusion: Despite performing prophylaxis for RhD alloimmunization by administering anti-D immunoglobulin to RhD negative women during pregnancy and after the birth of an RhD positive child, antigen RhD still represents the 2nd most frequent cause of maternal erytrocyte alloimmunization. The remaining clinically significant alloimmunizations are caused by non-D antigens of the Rh system, antigens of the Kell system, and rarely observed antigens of the MNS and Kidd blood systems.
