Předkládaná práce má dvě části, teoretickou a experimentální. První kapitola je věnována okluzi v přirozeném chrupu. Jsou v ní uvedeny definice vážící se k okluzi. Dále se kapitola podrobně zabývá problematikou ideální, fyziologické a stabilní okluze z pohledu několika autorů. Je také diskutováno téma centrálního vztahu čelistí. Jsou uvedeny různé definice této polohy, metody její registrace a vztah centrálního vztahu čelistí k maximální interkuspidaci. Dále jsou charakterizovány ideální okluzní kontakty při exkurzních pohybech dolní čelisti. Jsou zde uvedeny také jejich klasifikace uvedené v monografiích nebo studiích různých autorů. Kapitola se také zabývá problematikou okluzních překážek jejich definicí, rozdělením, výskytem v populaci, etiopatogenezí, klinickými projevy, diagnostikou, terapií a prevencí. Druhá kapitola se věnuje okluzi u různých typů protetických náhrad. Jsou zde uvedeny specifika okluze u fixních náhrad, částečných snímatelných náhrad, celkových snímatelných náhrad a náhrad na implantátech. Druhou část práce tvoří experimentální šetření. Studie popisuje vybrané parametry okluze u mladých dospělých studentů zubního lékařství. Konkrétně se zabývá vztahem maximální interkuspidace a centrálního vztahu čelistí, výskytem centrických interferencí a dvojího skusu. Také popisuje okluzní kontakty při protruzním pohybu, typická okluzní schémata při tomto pohybu a výskyt protruzních interferencí. Dále se věnuje popisu okluzních kontaktů při laterotruzním pohybu, typickým okluzním schématům při tomto pohybu a výskytem laterotruzních interferencí. Cílem práce je zejména odpovědět na důležitou klinickou otázku: jakou dynamickou okluzi zvolit u pacienta, u kterého nejsou zachovány původní okluzní vztahy, nebo u kterého je nutné z různých důvodů tyto vztahy změnit. Soubor tvořilo 100 studentů zubního lékařství. Na základě našich výsledků jsme došli k několika závěrům. Protruzní pohyb je obvykle veden přes frontální zuby. Okluzní kontakty na laterálních zubech při protruzním pohybu se vyskytují zřídka. Laterotruzní pohyb je veden obvykle pouze zuby pracovní strany. Nejčastějším způsobem vedení je vedení přes skupinu frontálních a laterálních zubů, následované vedením špičákovým a anteriorním. Až u třetiny studentů je možné nalézt kontakty na pracovní i nepracovní straně. Hyperbalanční kontakty jsou zcela výjimečné.This thesis has two parts, theoretical and experimental. The first chapter is devoted to occlusion in natural dentition. It presents definitions related to occlusion. The chapter defines ideal, physiologic and stable occlusion as viewed by different authors. Centric relation is also discussed. Various defintions of this position are reported, together with different methods of its registration. The relationship between centric relation and maximum intercuspal position is discussed. Ideal occlusal contacts during eccentric mandibular movements are characterized. Different classifications of occlusal contacts are presented. The chapter also deals with occlusal interferences: their definitions, types, prevalence, ethiopathogenesis, clinical manifestation, diagnostics, therapy and prevention. The second chapter is devoted to occlusion of different types of prosthetic reconstructions. Differences in occlusion of fixed reconstructions, partial removable dentures, complete removable dentures and implant-borne prosthetic reconstructions. In the second part, experimental investigation is reported. The study describes defined occlusal parameters in young adults dentistry students. Firstly, it deals with the relationship between centric relation and maximal intercuspal position, the prevalence of centric interferences and dual bite. Secondly, it describes occlusal contacts during protrusion, typical occlusal contact patterns during this movement and the prevalence of protrusive interferences. Thirdly, it describes occlusal contacts during laterotrusion, typical occlusal contact patterns during this movement and the prevalence of laterotrusive interferences. The aim of the study is to give the answer to the important clinical question: how to design occlusal contacts during eccentric mandibular movements of a patient, who has either lost his/her natural occlusal relationship or in whom this relationship must be changed. One hundred dental students were examined. On the basis of our results, we came to these conclusions. During protrusive movement, frontal teeth are usually in contact. Lateral teeth contact occasionally. During laterotrusion, only teeth of the working side are usually in contact. The most prevalent occlusal pattern is contact of both frontal and lateral teeth, followed by canine protected articulation and anterior protected articulation. In one third of students, occlusal contacts on both the working and the non-workung side were found. Occlusal contacts on the non-working side that disclude the working side were rare.
