1. fáze výzkumu měla za úkol zmapovat znalosti a deklarované zvyklosti (využívané intervence) všeobecných sester v oblasti péče o osoby s nehojící se ránou a bolestí. Cílem 2. fáze výzkumu bylo zjistit, jaké zkušenosti mají pacienti s akutní a chronickou bolestí při výskytu rány, a zda tyto zkušenosti korespondují s určujícími znaky ošetřovatelské diagnózy akutní a chronická bolest dle NANDA Int. (NANDA Int. 2012-2014). Záměrem 3. fáze výzkumu bylo zjistit, které aktivity NIC intervence "Pain management" jsou sestrami konzultantkami v hojení ran považovány na významné a zároveň uplatnitelné v klinické praxi. Metoda 1. fáze výzkumu: kvantitativní prospektivní výzkumná metoda pomocí dotazníkového šetření. Účelovým výběrem bylo osloveno 209 sester pracujících na pozici konzultantek pro hojení ran z 12 zdravotnických zařízení České republiky. V 2. fázi výzkumu byl využit model validity podle Fehringa validizace diagnózy na pacienta zaměřeným modelem klinické validity "Patient focus CDV model" ošetřovatelské diagnózy dle NANDA International "akutní a chronická bolest". Výzkumný soubor tvořilo 180 pacientů s akutní a chronickou bolestí při výskytu ran různé etiologie. 3. fáze výzkumu: kvantitativní sběr dat pomocí dotazníkového formuláře. Jednotlivé aktivity intervence NIC "Pain management" byly hodnoceny pomocí pětibodové Likertovy škály z pohledu expertů ve wound managementu (N = 188). V 1. fáze výzkumu bylo zjištěno, že dostupnost záznamu o bolesti spojeného s výskytem rány se liší podle typu pracoviště (p = 0,000) a že nejčastěji se jedná o záznam v běžné ošetřovatelské dokumentaci. Hodnocení bolesti sestrou probíhá nejčastěji, pokud pacient signalizuje bolest a má indikovánu analgetickou terapii. Za klíčové intervence v ošetřování pacienta s ránou a bolestí považují sestry získání anamnézy bolesti, zhodnocení příčiny bolesti a její řešení, zhodnocení efektu analgetické léčby a jejích pozitivních vlivů. Ke snížení bolesti sestry využívají nefarmakologické intervence. 2. fáze výzkumu ukázala, že hodnoty váženého skóre nad 0,8 byly zjištěny u pacientů s intenzitou bolesti uváděnou v rozmezí 7-10 bodů (na desetistupňové škále). Za těchto podmínek byly pacienty jako nejvýznamnější určující znaky vybrány: "vyhledávání úlevové polohy, ochranné chování, pozorované známky bolesti a častá bolest". Celkově definující charakteristiky ošetřovatelské diagnózy "chronická bolest" dosáhly nižších hodnot váženého skóre, ale jednoznačně byly zdůrazněny určující znaky typické pro dlouhodobé potíže jako je nehojící se rána: "strach ze vzniku další rány, omezená pohyblivost v místě bolesti, pozorované ochranné chování a trvalá bolest". 3. fáze výzkumu: Nebyl nalezen statistický rozdíl mezi hodnocenou významností a využitelností intervencí v klinické praxi. Znalosti o nefarmakologických intervencích a jejich využití, či případné propojení s ošetřovatelskými aktivitami u pacienta s bolestí při výskytu rány jsou stále nedostatečné, a že pro jejich realizaci není časový prostor s ohledem na organizaci práce v klinické praxi. Na základě zjištěných informací empirické části disertační práce, jednotlivých aktivit NIC intervence "pain management" a "wound care" a intervencí uváděných v sedmnácti analyzovaných odborných zdrojích z elektronických databází byl připraven doporučený postup intervencí u pacientů s bolestí a nehojící se ránou.The first phase of the research was supposed to map the knowledge and declared habits (used interventions) of general nurses taking care of patients with non-healing wound and pain. The aim of the second phase of the research was to find the experiences of the patients with acute and chronic pain when having wound and if this experience corresponds with defining signs of the nursing diagnoses acute and chronic pain according to NANDA Int. (NANDA Int. 2012-2014). The third phase of the research was concentrated on what activities of NIC interventions: Pain management" nurses - wound consultant considered to be important in healing wounds and applicable in clinical practice as well. The Method the first phase of the research used a quantitative prospective research method (questionnaire survey). We selected 209 nurses (wound consultants) in 12 health care facilities in the Czech Republic. In the second phase a model of validity according to Fehring was used - validation of diagnosis per patient oriented clinical model validity "Patient focus CDV model" nursing diagnoses according to NANDA International "acute and chronic pain". The research group consisted of 180 patients with acute and chronic pain having wounds of different aetiology. In the third phase of the research we acquired quantitative data using questionnaire forms. Each activity of the NIC "Pain management" was evaluated with the five - point Likert scale from the point of view of experts in wound management (N = 188) In the first phase of the research we found out that the availability of the record the pain connected with wounds differs according to the type of the workplace (p = 0,000) and that the record mainly deals with nursing documentation. In general defining characteristics of the nursing diagnoses the chronic pain have reached lower values, but what was really emphasise were the defining signs typical for the long term problems such as: limited mobility at the locality of a pain, the observed protective behaviour, chronic pain". In the third phase of the research was not find statistical differences between a significance and utilisation of the interventions in the clinical practice. In general defining characteristics of the nursing diagnoses the chronic pain have reached lower values, but what was really emphasise were the defining signs typical for the long term problems such as: limited mobility at the locality of a pain, the observed protective behaviour, chronic pain". In the third phase of the research was not find statistical differences between a significance and utilisation of the interventions in the clinical practice. the connection of the nursing activities with a patient with wound and pain, is still insufficient and there is no time for their realisation with regard to the organization of work in clinical practice. Conclusion: Based on the acquired information of the empiric part of the dissertation work, individual activities of NIC interventions "pain management" and "wound care" and interventions mentioned in 17th analysed special resources from electronic databases we have created a recommended procedure (clinical recommendation) for patient with pain and non- healing wound.
