Kašel je jedním ze základních fyziologických mechanismů, díky kterému je zajištěna hygiena dýchacích cest. U pacientů s výrazným oslabením dýchacích svalů mohou být postiženy všechny 3 fáze kašle: inspirační, glotická i exspirační. V rámci rehabilitace a hygieny dýchacích cest lze volit buď konvenční airway clearance techniques, nebo speciální techniky založené na podpoře nádechových i výdechových svalů. Konkrétní technika je volena v závislosti na míře oslabení dýchacích svalů a poškozené fázi kašle. U pacientů, kde výrazné oslabení dýchacích svalů již neumožňuje aplikovat aktivně prováděné drenážní techniky, je možné použít přístrojovou podporu pomocí mechanické insuflace-exsuflace, kterou lze použít u širokého spektra onemocnění. Je založena na střídání přetlaku a podtlaku, čímž dochází k simulaci kašle a zefektivnění expektorace.Cough is one the basic physiological mechanisms which ensures the hygiene of the airways. In patients with considerably weak respiratory muscles, all three phases of the cough cycle can be affected, i.e. the inspiratory phase, the compression phase, and the expiratory phase. As far as the rehabilitation and hygiene of the airways are concerned, it is possible to apply either the conventional airway clearance techniques, or special techniques based on supporting the inspiratory and expiratory muscles. The particular technique is selected according to the level of respiratory muscle weakness, depending on which cough cycle phase is affected. If the active drainage techniques cannot be applied due to the significant weakness of the patient's respiratory muscles, it is possible to use amechanical insufflation-exsufflation device, which can be applied to a wide range of diseases. It is based on the positive and negative pressure alternation which stimulates cough and makes expectoration more effective.
