Karcinom prsu je nejčastější nádorové onemocnění postihující ženskou část populace. Jedním ze sledovaných markerů při volbě vhodné terapie je množství receptoru HER2. U cca 15 ? 20 % pacientek s karcinomem prsu dochází k amplifikaci a/nebo k nadměrné expresi genu Her-2/neu. Stupeň amplifikace koreluje s vyšší agresivitou nádoru a horší prognosou, proto je nezbytné před zahájením terapie stanovit status genu HER2. Pro léčbu HER2 pozitivních karcinomů prsu lze po selhání léčby trastuzumabem podávat lapatinib, tyrosinkinasový inhibitor receptoru HER1 i HER2. Práce je zaměřena na vztah léčebné odpověď pacientek s karcinomem prsu ke stavu receptorů HER1 a HER2 a plasmatickým hladin lapatinibu. Experimentální část diplomové práce je zaměřena na vývoj, validaci a stanovení plasmatických hladin lapatinibu pomocí ultra účinné kapalinové chromatografie ve spojení s tandemovou hmotnostní spektrometrií. Vývoj metody spočíval v nalezení optimálních MS/MS pro detekci a kvantifikaci analytu. Vyvinutá metoda byla validovaná pro budoucí použití v klinické praxi (linearita, návratnost, nepřesnost a reprodukovatelnost). Následně byla metoda aplikována na reálné vzorky pacientek léčených lapatinibem. Byla prokázána souvislost mezi pozitivitou receptoru HER2 a léčebnou odpovědí pacientek. U podskupiny pacientek se změnami receptoru HER1 nebyl nalezen statisticky významný vztah pozitivity HER1 receptoru a léčebné odpovědi na lapatinib. Nově vyvinutá metoda je vysoce účinná, časově nenáročná a po validaci použitelná v rutinní praxi.Breast cancer is the most frequent tumor disorder attacking the female part of the population. One of the monitored markers for selection of the acceptable therapy is the amount of the HER2 receptor. Around 15-20 % of patients with the breast cancer amplificated and/or over expressed the Her-2/neu gene. Degree of amplification correlates with higher agressivity of the tumor and worse prognosis; therefore it is important to determine the status of the HER2 receptor before the initialization of the therapy. It is possible to indicate lapatinib, the tyrosinkinase inhibitor of the HER1 and HER2 receptor, for HER2-positive breast cancer therapy after the failure of the trastuzumab treatment. The aim of this work is to investigate the relation between the therapy response of the patients with the breast carcinoma to the HER1 and HER2 receptors situation and the plasma levels of the lapatinib. The aim of of the experimental part of this thesis is to the develop, validate and determinate of plasma levels of the lapatinib by using of ultra high performance liquid chromatography coupled with tandem mass spectrometry. The method development consisted of finding of the optimal conditions for MS/MS detection and quantification of the analytes. The developed method was validated for next usage in clinical practice (linearity, recovery, accuracy and reproducibility). The method was applied on real samples of patients under the lapatinib therapy. The relationship between positivity of the receptor situation and the therapy response was found. In subgroup of patients with HER1 receptor changes any statistical important relation between HER1 receptor positivity and therapy response on the lapatinib was found. The newly developed method is effective, time?unconsuming and suitable for routine use after the validation.
