Diplomová práce je zaměřena na problematiku využití handlingu v terapii pacientů s hemiparézou. V rámci Bobath konceptu chápeme handling jako specifickou terapeutickou manipulaci s pacientem, přičemž jeho techniky jsou uplatňovány při každém pohybu a transferu pacienta. Teoretická část shrnuje základní poznatky o lidské bipedální lokomoci včetně mechanismů podílejících se na jejím řízení, další část je zaměřena na specifika chůze hemiparetických pacientů. Zvláštní kapitola se pak věnuje základním principům terapeutického handlingu včetně jeho uplatnění v terapii chůze pacientů po iktu. Cílem experimentální části práce bylo pomocí moderní kineziologické přístrojové techniky posoudit, zda a do jaké míry jsou pacienti schopni přenést aktivitu facilitovanou terapeutem bezprostředně do samostatného provedení dané funkce, v našem případě chůze. Výsledky práce naznačují, že umožnění prožití normálního pohybu může ovlivnit především způsob provedení dané aktivity a tím optimalizovat energetické nároky již těsně po terapii a poskytuje nám argumenty pro využití terapeutické facilitace v terapii nejen u hemiparetických pacientů.The thesis is focused on problems of usage of the therapeutic handling in rehabilitation of patients with hemiparesis. Within the frame of the Bobath concept handling can be understood as a specific therapeutic manipulation with the patient, whereas its techniques are applied on every patient's movement and transfer. The theoretical part of the thesis summarizes the fundamental findings about human bipedal locomotion including its control mechanisms, the next part focuses on the gait specifics of hemiparetic ill patients. Separate chapter deals with fundamental principles of the therapeutic handling including its use in the therapy of the patients after stroke. The aim of the experimental part was to examine if and to what degree the patiens are able to transfer the activity facilitated by a therapist immediately to the independent performance of the specific function, in this case, to the gait with the aid of the modern kinesiological instrumentation. The results suggest, that through enabling normal movement experience, we can influence first of all the movement itself and thus optimize the energy costs already immediately after a therapy session. This provides arguments for the usage of the therapeutic facilitation in rehabiliation not only by hemiparetic patients.
