Dizertační práce byla zaměřena na hledání asociace mezi aloimunizací proti erytrocytovým antigenům a přítomností konkrétních variant HLA II. třídy. Do studie bylo zařazeno 230 responderů a 109 pacientů bez aloprotilátek (nonresponderů). Respondery jsme dělili na monorespondery tvořící protilátky proti jednomu antigenu a multirespondery tvořící protilátky proti alespoň dvěma antigenům, Mezi nejvýznamnější nálezy práce patří nezávislé potvrzení vztahu varianty HLA-DRB1*15 s tvorbou vícečetných RBC protilátek a identifikace dalších asociací HLA II. třídy pro tři časté kombinace protilátek. Nalezené asociace s HLA-DRB1*15 a DRB1*13 byly potvrzeny i při analýze podle Rh fenotypů, což by perspektivně mohlo pomoci identifikovat pacienty ve vyšším riziku RBC aloimunizace.This thesis was focused on identification of an association between alloimmunization of red blood cell (RBC) antigens and HLA Class II variants. In total, 230 responders and 109 patients without RBC alloantibodies (non-responders) were enrolled in the study. Responders were divided into mono-responders forming monospecific alloantibodies and multi-responders forming at least two antibody specificities. The most important findings of the work was independent confirmation of the relationship of the HLA-DRB1 *15 variant with the production of multiple RBC antibodies and identification of other HLA Class II associations for three common antibody combinations. Demonstrated associations with HLA-DRB1*15 and DRB1*13 were also confirmed in the sub-analysis according to the patients' Rh phenotype, which could potentially help identify patients at higher risk of RBC alloimmunization.
