U traumatických poranění kolenního kloubu, vedle poškození jiných anatomických struktur, velmi často vznikají defekty kloubní chrupavky, které narušují hlavní funkce kloubu. Pro fungování kloubu je nejdůležitější udržování kongruence kloubních ploch a odolnost vůči opakovanému zatížení. Poranění v místě zátěžových ploch kloubu vede po zatížení ke vzniku bolesti, která negativně ovlivňuje každodenní život pacienta. Většina lézí je řešena chirurgickým výkonem, zvoleným podle typu pacienta a vlastností defektu. Teoretická část práce poskytuje souhrn poznatků o složení, struktuře a biomechanických vlastnostech kloubní chrupavky, dále popis chondrálních a osteochondrálních lézí s důrazem na jejich diagnostiku, vyšetření, konzervativní, chirurgickou a rehabilitační léčbu. V praktické části je popsána kazuistika pacientky s traumatickou lézí chrupavky kolenního kloubu řešenou technikami stimulujícími kostní dřeň, transplantací osteochondrálních štěpů, transplantací autologních chondrocytů i konzervativními metodami.Traumatic injuries of a knee joint often lead to articular cartilage lesions. The main function of articular cartilage is to provide a smooth and congruent articular surface, which is resistant to repetitive stress. If the articular cartilage is damaged in the load zone of the joint, it may cause pain, which negatively affects patient's activities of daily living. The majority of lesions are treated by a surgical approach. A choice of the surgical method is based on a type of a patient and characteristics of the defect. In the theoretical part of this thesis there is a summary of information about histology, structure, biomechanical characteristics of the articular cartilage, description of chondral and osteochondral lesions with an emphasis on their diagnostics, examination, conservative, chirurgical and rehabilitative treatment. In the practical part of this work there is a case report of a patient with the chondral lesion of traumatic etiology, which was treated by marrow stimulation techniques, osteochondral transplantation and conservative methods.
