Jaterní fibróza je jedním z nosných témat současné hepatologie. Fibróza a možná i cirhóza jsou podle posledních poznatků reverzibilní. Uvedená práce je zaměřena na problematiku diagnostiky fibrózy u pacientů s chronickou hepatitidou C. Shrnuje základní poznatky o fibróze, fibrogenezi, diagnostice jaterní fibrózy. Jsou uvedeny neinvazivní diagnostické postupy: sérové markery, dechové testy, zobrazovací metody, měření tuhosti jaterní tkáně. Vlastní práce porovnává stupeň-staging-zjištěný histologicky s výsledky sérových testů u74 pacientů s chronickou hepatitidou C. Byly užity testy-deRitisův kvocient, Fornsův test, APRI, Fibrotest, Fibroindex, Fib-4, Fibrofast, CDS, AP index. V další části práce byl také posuzován vztah fibrózy a kouření u sledovaných pacientů, zda faktor kouření ovlivňuje výsledky sérových markerů fibrózy.Liver fibrosis is one of the principal topics of present hepatology. Fibrosis and maybe even cirrhosis are according to the recent knowledge reversibile. The presented work is focused on problems of diagnosis of fibrosis in patients with the chronic hepatitis C. The work sums up the basic knowledge about liver fibrosis, fibrogenesis.Non-invasive diagnostic methods: serum markers, breath tests, imaging methods, measurement of liver stiffness are presented. The work itself compares staging,which was described by histopathologist, with results of serum tests in 74 patients with chronic hepatitis C.We used the following tests: de Ritis ratio, Forns?s test, APRI, Fibrotest, Fibroindex, Fib-4, Fibrofast, CDS, AP-index. We evaluated the relation of fibrosis and cigarette smoking in pacients in the cohort, whether factor of smoking influences results of serum markers or not.
