Bakalářská práce se zabývá individuálním plánováním v kontextu nízkoprahového zařízení pro děti a mládež. V teoretické části charakterizuje tato zařízení, zasazuje je do kontextu sociálních služeb a definuje jejich poslání, cílovou skupinu a průběh poskytování této služby. Dále se věnuje vymezení období adolescence, jeho typickým znakům a také rizikovému chování adolescentů, které je častým tématem v praxi nízkoprahového zařízení pro děti a mládež. Cílem výzkumu bylo navržení kvalitního a efektivního formuláře pro zaznamenávání individuálních plánů klientů těchto zařízení. Výzkum obsahuje dvě části: analýzu formulářů pro zaznamenávání individuálních plánů 12 nízkoprahových zařízení pro děti a mládež a kvalitativně orientovaný dotazník pro jejich pracovníky. Výsledný návrh formuláře bude nabízen nízkoprahovým zařízením pro děti a mládež v rámci České republiky.This bachelor thesis deals with individual planning in the context of youth drop-in centres. The teoretical part of the thesis describes these facilities, sets them in the context of social services and defines their function, target group and the process of providing this service. It also discusses the definition of adolescence, the typical characteristics of this period and risk behavior of adolescents. Which, in practice is a common issue of youth drop-in centres. The goal of the research was to design a quality and effective form for recording individual plans of twelve youth drop-in centres and qualitatively oriented questionnaires for employees of this centres. The final design of this form will be offered to youth drop-in centres the Czech Republic.
